Biofilm formation is essential for the survival and growth of Escherichia coli in catheter-associated infections. Individuals with type 2 diabetes mellitus can excrete insulin and/or glucose in their urine. This population also has an increased incidence of urinary tract infections. The focus of this study was to determine if the composition of Foley catheter material affects biofilm formation by E. coli in a model system for type 2 diabetes mellitus. Rubber (lubricious-coated), silicon-coated, silver-coated and nitrofurazone-coated catheter segments (5 mm; n = 6) were tested. Catheter segments were added to E. coli ATCC25922 (10 4 CFU/ml, final concentration) in artificial urine alone, or with insulin (40 µU/ml) and/or glucose (0.1%). After incubation (18 h, 37˚C, in air and anaerobically) the level of catheter-associated biofilm was determined by crystal violet staining (Abs 550nm ). Statistical analysis was done by ANOVA with post-hoc analysis (Tukey). Neither nitrofurazone-coated nor silver-coated catheters supported the formation of E. coli biofilm, regardless of growth condition tested. In contrast, under aerobic biofilm formation on silicon catheters was significantly higher (p < 0.05) than that on sterile catheter alone. In addition, glucose with insulin induced significantly more biofilm (p < 0.05) than E. coli controls. Biofilm formation was also significantly increased (p < 0.05) under anaerobic conditions on lubricious-coated rubber catheters as compared to sterile catheters. These results may aid in the development of a catheter material that can prevent biofilm formation, or alternatively guide choice of catheter material for individuals shedding insulin in their urine.

PURPOSE OF REVIEW: Urinary incontinence in male neurological patients is a very frequent problem but treatment remains challenging. Thus, we summarize and highlight the latest developments in the management of urinary incontinence in this specific patient population. RECENT FINDINGS: Intermittent self-catheterization, antimuscarinics, intradetrusor injections with onabotulinumtoxinA, augmentation cystoplasty, urinary diversion, and artificial urinary sphincter are the cornerstones of the armamentarium for treating neurogenic urinary incontinence. However, with the exception of onabotulinumtoxinA intradetrusor injections, level of evidence is often low and male-specific outcomes are virtually not available. Alternative conservative and/or minimally invasive procedures such as neuromodulation techniques and suburethral suspension devices provide promising data with apparently good safety and tolerability but still insufficient evidence lacking randomized control trials. SUMMARY: Standard options for treatment of urinary incontinence in neurological patients remain largely unchanged. Alternative treatment options, especially of conservative or minimally invasive character, have the potential to further broaden the therapeutic spectrum. While a higher level of evidence is needed to assess the potential of such therapeutic approaches, randomized controlled trials in the male neurological population present a challenge. To truly advance treatment of urinary continence in male neurological patients, well-designed, multicenter studies are warranted. Este estudo teve como objetivo avaliar a conduta do enfermeiro frente ao traumatismo de uretra ocasionado pela inserção do cateter uretral. Este estudo teve aprovação do Comitê de Ética em pesquisa da Escola de Enfermagem de Ribeirão Preto -Parecer 466/12 e foi realizado em um Hospital Universitário em duas etapas: na primeira etapa, descritiva, foi efetuada consulta ao sistema eletrônico do hospital que trata de notificações sobre eventos adversos dos pacientes. Nessa etapa foi utilizado instrumento de perguntas objetivas elaborado pelo próprio pesquisador. Na segunda, quase-experimental pós teste, foram coletados os dados relacionados a autoconfiança de enfermeiros, que participaram de cenário simulado de baixa fidelidade. Nessa etapa para obtenção dos dados foi aplicado um questionário de caracterização dos sujeitos e instrumento já validado, de autoconfiança na Assistência de Enfermagem à Retenção Urinária (EAAERU), a qual encontra-se dividida em cinco fatores: 1) "Intervenções realizadas durante o cateterismo urinário e/ou em situações iatrogênicas", 2) "Intervenções prévias ao cateterismo urinário", 3) "Intervenções realizadas após o cateterismo urinário"; 4) "Comunicação, consentimento e preparo dos materiais para realização do cateterismo urinário", e 5) "Avaliação objetiva da RU" (retenção urinária). Os dados da primeira etapa foram analisados por estatística descritiva e os dados da segunda etapa foram codificados e digitados duplamente em planilhas do aplicativo Excel®, exportados e analisados no programa SPSS (Statistical Package for Social Science) ®, versão 22.0, como a escala original. Os resultados demonstram que na primeira fase do estudo entre as 5300 notificações do serviço, apenas 27 (1,96%) estavam relacionadas a problemas urinários. Entre essas, cinco diretamente ao cateterismo urinário. Na segunda fase do estudo, a amostra foi composta por 53 enfermeiros, entre os quais a maior parte era do sexo feminino, com idade média de 36 anos, menos de dez anos de formação. Entre eles a maioria cursava ou já haviam cursado pós-graduação. Segundo os entrevistados todos 53 (100,0%) já realizaram o cateterismo urinário e 46 (86,8%) já vivenciaram dificuldades no procedimento.
