Complications of double balloon enteroscopy: a multicenter survey

Abstract: Diagnostic DBE is safe with a low complication rate. The complication rate of therapeutic DBE is high compared with therapeutic colonoscopy. The reason for this is unclear. The incidence of pancreatitis after DBE is low (0.3 %), but has to be considered in patients with persistent abdominal complaints after a DBE procedure.

“…Whether these patients required hospitalization is not stated. The multicenter questionnaire survey by Mensink et al identified 6 cases of pancreatitis in 1728 diagnostic procedures (0.3%) and 1 case of pancreatitis after DBE-assisted ERCP in a patient with a Roux-en-Y reconstruction [16] . Several possibilities should be considered when trying to explain the difference between post-DBE pancreatitis rates in the present study and the previously published studies.…”

“…DBE plays an important role in the diagnosis and treatment of obscure gastrointestinal bleeding [3][4][5][6] , Crohn's disease [7][8][9][10] , celiac disease [11,12] , polyposis syndromes [13] and small bowel tumors. In the series published to date, complications solely related to the diagnostic procedure are rare [14][15][16][17][18] . A recent retrospective multicenter survey indicates a complication rate of 0.8% in diagnostic procedures [16] .…”

“…12 However, there is an increased risk of perforation with balloon dilatation, especially in the cases with acute inflammation. 12,13 In our case, we only used the over tube to gently dilate the stricture in order to minimize the risk of perforation. Retrograde enteroscopy may offer an alternative option for the removal of retained capsule.…”

“…The AEs rate is higher for therapeutic (4.3%) than for diagnostic DBE (0.8%). The rate of bleeding or perforation may be as high as 10.8% for patients undergoing polypectomy during DBE [67,68].…”
