Objective: The flashlamp pulsed dye laser (FLPDL) is currently the treatment of choice for port-wine stains (PWS). We recently investigated whether a non-coherent intense pulsed light source (IPLS) would be effective in the therapy of PWS. Design and Patients:In order to evaluate the efficacy in treatment of PWS (especially adult-type dark and hypertrophic), a retrospective study of 37 patients (randomly selected) with a total of 40 PWS treated with IPLS was initiated. Clinical PWS characteristics recorded were color and location of the PWS. Data collected included treatment parameters (filters, pulse duration, fluence and pulse sequencing), % clearance, and side effects (purpura, blisters, crusting, altered pigmentation and scarring).Results: Good and complete (70-100 %) clearance was achieved in 28 of 40 PWS treated with IPLS. Average number of treatment sessions in PWS reaching 100 % clearance included 4.0 for pink PWS and 1.5 sessions for red PWS. Average number of sessions in purple PWS reaching good clearance (70-99%) was 4.2 sessions. Parameters used most frequently were 515 and 550 nm cut-off-filters, pulse duration of 2.5-5.0 ms and fluences of 24 to 60 J/cm 2 . Side effects included purpura in 76%, superficial blisters in 8% and crusting in 20%. Transient pigmentation changes were seen in 10.8% of patients (hypopigmentations in 8.1%, hyperpigmentation in 2.7%). No scarring was observed. Conclusion:IPLS presents an effective and safe method for treating PWS, especially dark and facial PWS.

Hypertrophic scars are common and cause functional and psychologic morbidity. Various treatment modalities have been advocated in the past, but the flashlamp-pumped pulsed-dye laser-585 nm (PDL) has been shown to be effective in the treatment of a variety of traumatic and surgical scars. This study was performed to determine the effectiveness of the PDL treatment of hypertrophic scars in Asians and to elucidate possible side effects and complications. Thirteen Asian patients with 19 hypertrophic scars that had no response to conservative therapies, such as pressure therapy, intralesional corticosteroid injection, or silastic gel sheeting for more than 6 months, were treated with the PDL. Three months after the last laser treatment, improvement in the appearance, reduction in scar height, and erythema as a result of the PDL treatment were evaluated and compared with baseline values. Compared with the baseline appearance, hypertrophic scars treated with the PDL showed a satisfactory clinical improvement of 84% (16 of 19), a significant percentage of scar flattening of 58.9 +/- 22.1%, and of erythema elimination of 75.7 +/- 19.9% at month 3 after the last laser treatment. The PDL treatment of hypertrophic scars can effectively improve scar pliability and texture and decrease erythema and associated symptoms. In addition, multiple treatment sessions are suggested for achieving greater response, but lower fluences are recommended for prevention of side effects in Asians with a higher melanin content of the skin.

Many different laser systems are used in dermatology. To wisely choose the correct laser for a given problem, one must be aware of both the spectrum of disorders for which each laser is suited and the potential side effects. We compare the side effect and complication profiles of the common laser systems pointing out their possibilities and limitations. Typical treatment errors will be pointed out. Dermatological training, extensive experience in laser therapy and compliance with quality guidelines are prerequisites for safe and successful treatment.
