Complication Rates After TRUS Guided Transrectal Systematic and MRI-Targeted Prostate Biopsies in a High-Risk Region for Antibiotic Resistances

Abstract: Conclusion: Complications after transrectal prostate biopsies are rare and often self-limiting. Infections were seen in <1% of all patients, regardless of an elevated local prevalence of MDROs. Severe complications (Clavien-Dindo ≥ IIIa) were only seen in 3 (1.7%) of the patients. Repeated biopsy is associated with higher complication rates in general.

“…Thus, complication rates in a transperineal approach with cefpodoxime prophylaxis might be even lower. However, other studies also suggested that a transrectal approach is even safe in regions with increasing proportions and high risk of antibiotic resistance [10].…”

“…The main noninfectious complications reported after prostate biopsies are hematuria, hematospermia, urinary retention, and rectal bleeding [9]. Antibiotic resistance, especially against fluoroquinolones, has been increasing in recent years [10,11].…”

“…However, complications such as hematuria, hemospermia, rectal bleeding, and high fever, most of which are not serious regress spontaneously without treatment. Hematuria and hemospermia are more common than others [2,4,10]. In their review, Borghesi et al [10] indicated that the factors affecting the occurrence of hematuria were releated with method, prostate volume, and comorbidities in prostate biopsy.…”

“…The most serious and feared complication is urosepsis. Urosepsis, which is a serious but rare complication, was observed between 0.1% and 2.8% in the literature [3][4][5][6]. Pre-and post-biopsy prophylactic oral quinolone is recommended to prevent these infectious complications [2].…”

“…Men with elevated serum PSA are sent for transrectal ultrasound (TRUS)guided biopsy for diagnosis and grading of PCa (3). Considered a generally safe and welltolerated outpatient procedure (4), needle biopsy is potentially associated with complications affecting patients' quality of life to various degrees (5,6). Unfortunately, over 50% of biopsies performed on men with elevated PSA were cancer free (7), and 30-50% of the detected cancers were low grade (8).…”
