Complicated pregnancies in inherited distal renal tubular acidosis: importance of acid-base balance

Abstract: Inherited distal renal tubular acidosis (dRTA) is caused by impaired urinary acid excretion resulting in hyperchloremic metabolic acidosis. Although the glomerular filtration rate (GFR) is usually preserved, and hypertension and overt proteinuria are absent, it has to be considered that patients with dRTA also suffer from chronic kidney disease (CKD) with an increased risk for adverse pregnancy-related outcomes. Typical complications of dRTA include severe hypokalemia leading to cardiac arrhythmias and paralys… Show more

“…Due to the high heterogeneity of CKD, we do not know if and how pregnancy outcomes are modulated by the different nephropathies, as besides the most common ones such as IgA or lupus nephropathy, diabetic nephropathy, and reflux nephropathy, evidence is scant. 44,45,[112][113][114] How should we define preconception risks of pregnancy with respect to current proteinuria cut offs? Indications on when to start dialysis in pregnancy are not well established, nor is the specific role of frequency and duration.…”

“…Доказательства, помимо каса-ющихся наиболее распространенных нефропатий, таких как IgA-нефропатия, волчаночный нефрит, диабетическая нефропатия и рефлюкс-нефропатия, крайне скудны [44,45,[112][113][114]. Как следует опреде-лять предгравидарные риски беременности в свете пороговых значений протеинурии?…”
