Complex Partial Seizures in Young Children

Abstract: We retrospectively analyzed the clinical manifestations of complex partial seizures (CPS) in children aged < or = 10 years using video and EEG telemetry and evaluated their course, investigations, management, and seizure status at follow-up. Seventeen patients with CPS were studied at the Prince of Wales Children's Hospital (POWCH) and Prince Henry Hospital (PHH) between 1987 and 1992. Because 15 of the 17 patients had intractable seizures, the population was selective. Mean age was 6.5 years; 4 patients were … Show more

“…As previously reported, TLE in childhood is not rare and can be recognized correctly even in very young children (Jayakar and Duchowni, 1990; Wyllie et al, 1993; Bye and Foo, 1994; Brockhaus and Elger, 1995; Harvey et al, 1997; Rathgeb et al, 1998; Broglin, 2001; Franceschetti et al, 2001; Pachatz et al, 2001; Fogarasi et al, 2002; Franzon et al, 2004; Ray and Kotagal, 2005). In our population, age at onset was < 6 years in 82% and < 3 years in 51% of cases.…”

“…In children, however, the clinical features of temporal lobe seizures (TLS) are controversial. Most reports on CPS in children have included cases of extratemporal as well as temporal seizure origin (11)(12)(13)(14)(15)(16)(17)(18)(19).…”

“…In previous studies of seizure classification in infants based solely on symptoms, we (2,3) and other authors (4–10) noted a common seizure type characterized mainly by decreased behavioral activity with indeterminate level of consciousness. These have been called hypomotor (2,3) or behavioral/motion arrest (6,8–10) seizures.…”

“…In previous studies (2)(3)(4)(5)(6)(8)(9)(10)16,(21)(22)(23)(24)(25) of temporal lobe epilepsy in children, an age-related evolution of symptoms has been noted. Fine motor automatisms first appear in the preschool age (4,5,22), whereas automatisms in infants with temporal lobe epilepsy are usually limited to oral or simple gestural movements (2,6,9,10,16,24). We also noted an age-related trend in the current study, with hypomotor seizures seen most often in our youngest patients younger than 1 or 2 years.…”
