Comparison of Various Rhinoplasty Techniques and Long-Term Results

Abstract: The endonasal approach is an appropriate choice for select patients due to the lower chance of complications and a shorter procedure time. With open rhinoplasty with the transcolumellar incision technique, the type of surgery can be more easily controlled, however, extended nasal tip edema and columellar scar are some of the disadvantages of this method. ORWTI allows a patient to avoid these disadvantages and provides a more controlled procedure, similar to the open method.

“…The most generalizable studies 1,5 available in the rhinoplasty literature focus on cosmetic rhinoplasties from surgeons in a single practice or institution, with revision rates of 9.8% and 11%. One single-surgeon, single-institution study 2 published a revision rate of 4% with a sample size of 1524 patients undergoing cosmetic and noncosmetic rhinoplasty. In the literature focusing on specific rhinoplasty techniques, the revision rate varied from 4% to 15.5%.…”

“…8 During a rhinoplasty, the nasal skin, subcutaneous soft tissue, cartilaginous and bony framework, and mucous membrane lining are manipulated. 9 The open rhinoplasty is differentiated from the endonasal rhinoplasty in that the incision is made in the columella (fleshy tip of the nose that separates the nares) in the open approach. 9 The general principle of a rhinoplasty consists of a separation of the nasal skin and soft tissues from the osseocartilaginous nasal framework so that the framework can be reshaped to produce the desired nasal contours.…”

“…Thus, my indications for the open approach are narrower than in Dr. Ors practice. In the actual study, the open approach was most reliable in achieving good tip projection [1], but it was also associated with the presence of several drawbacks. They were well described by Constantian [9]: an increased morbidity, longer recovery, higher risk for circulatory disturbances in the columella and the always present columellar scar, even if its quality may vary.…”

“…This is particularly true with rhinoplasty, which can be executed by one of three approaches: endonasal or closed, semi-open using an infracartilaginous incision and open or external technique. The authors Safvet Ors, Mehmet Ozkoze and Sevgi Ors have performed a retrospective analysis of 1526 patients operated on from 1999 to 2013 [1]. All surgeries were done by the first author.…”
