Objective To investigate the extent to which primary-care practitioners routinely inquire about postpartum urinary and faecal incontinence, and assess the proportion of women who disclose symptoms.Design Prospective pregnancy cohort study of nulliparous women.Setting Melbourne, Australia.Sample A total of 1507 nulliparous women recruited in early pregnancy.Method Women were recruited from six public maternity hospitals, with follow up at 3, 6, 9 and 12 months postpartum.Main outcome measures Standardised measures of urinary and bowel symptoms, and measures of health service use.Results In the first 12 months postpartum, the period prevalence of urinary incontinence was 47%, and of faecal incontinence was 17%. In all, 86% of women visited a primary health-care practitioner at least once to discuss their own health in the first year after childbirth. However, only around a quarter were asked about urinary incontinence, and fewer than one in five women were asked about faecal incontinence. Discussion of symptoms with health professionals was most likely to occur in the first 3 months postpartum, and happened only rarely during the remainder of the first postnatal year. Over 70% of women reporting severe urinary incontinence and/or faecal incontinence had not discussed symptoms with a health professional. ConclusionThe findings provide robust evidence that many women experiencing postpartum urinary and faecal incontinence -including women with moderate and severe symptoms-do not receive adequate primary-care follow up in the first 12 months postpartum. Systems of maternal health surveillance need to include routine inquiry about urinary and faecal incontinence to overcome women's reluctance to seek help.

Método: Estudio transversal cuyo muestra incluyó 500 mujeres que comenzaron la atención prenatal en un servicio del sector de salud complementaria en Guarulhos, SP, en 2012-2013. La FMSP fue evaluada por la perineometría y las mujeres embarazadas con incontinencia urinaria (IU) respondieron al International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF). Resultados: Muestran que edad materna (OR=1,06 IC95% 1,02-1,11) y IU previa (OR=15,12; IC95% 8,(19)(20)(21)(22)(23)(24)(25)(26)(27)92) son las variables que, en conjunto, mejor explican la ocurrencia de IU al inicio del embarazo. Results: It was found that maternal age (OR=1.06; CI95% 1.02-1.11) and prior UI (OR=15.12; 95%CI 8.19-27.92) are the variables that, in tandem, best explain the occurrence of UI at the beginning of pregnancy. The mean score on the ICIQ-SF was 8.2 (SD=3.9), considered a moderate impact on quality of life. Introduction:We evaluate the impact of severe obesity on surgical outcomes of the transobturator tape (TOT) procedure in patients with stress urinary incontinence (SUI). Methods: In total, 32 women with severe obesity (body mass index [BMI] >35 kg/m 2 ) were included in the study. All patients were preoperatively evaluated with history, pelvic examination, ultrasonography, and cough stress test. All patients completed the International Consultation on Incontinence Questionnaire-Short form (ICIQ-SF) preoperatively and at the postoperative followup. Cure of incontinence was defined as being completely dry after surgery. Cure was assessed subjectively and objectively. Subjective improvement defined as an International Consultation on Incontinence Questionnaire-Short form (ICIQ-SF) score ≤12 and satisfaction with surgery. Failure was defined as having no change or worsening of urinary incontinence after surgery. Postoperative patient satisfaction was assessed using a visual analog scale. Results: The mean follow-up time and mean BMI were 40.9 ± 20.9 months and 38 ± 3 kg/m 2 , respectively. According to preoperative ICIQ-SF questionnaire scores, 20 patients (62.5%) had severe and 12 patients (37.5 %) had very severe urinary incontinence symptoms. No patient had slight or moderate symptoms. None of the patients experienced worsening symptoms after surgery. Objective cure, subjective cure, subjective improvement and patient satisfaction rates were 81.2%, 46.8%, 37.5%, and 84.3% respectively. Our overall complication rate was 9.3%. None of the patients experienced intraoperative complications. Conclusion: In experienced hands, TOT is an effective and safe procedure to treat SUI, with minimal complications in severe obese women.
