Comparison of the Incidence of Vancomycin‐Induced Nephrotoxicity in Hospitalized Patients with and without Concomitant Piperacillin‐Tazobactam

Abstract: We observed an increased incidence of nephrotoxicity in vancomycin-treated patients who received concomitant piperacillin-tazobactam. A steady-state vancomycin trough concentration of 15 μg/ml or greater was also associated with an increased risk of the development of nephrotoxicity. These findings should be confirmed in larger, randomized studies.

“…Burgess & Drew (2014) The present study has several limitations that must be discussed in combination with the results. The time frames for inclusion could have encompassed a larger date range with the same number of months included in the pre-implementation and post-implementation groups to account for seasonal variations in antibiotic-prescribing patterns.…”

“…Empiric antibiotic therapy often involves combinational agents which further increase risk of nephrotoxicity. 59,60 Furthermore, among patients with heart failure which commonly complicates IE, the risk of nephrotoxicity is heightened. 61 …”

“…45 This finding with piperacillin-tazobactam is not surprising as its nephrotoxicity risk appears to be more significant than previously appreciated. [46][47][48][49] We do not believe that the tobramycin was the cause of the AKI alone in our case, but rather a contributing factor. Our patient experienced significant post-operative hypotension for a brief time period and was also on intravenous vancomycin.…”
