Objectives The aim of this study was to evaluate the effectiveness of different fiber post removal techniques and to correlate dentinal loss with microcrack formation. Materials and methods Forty-five extracted single-rooted teeth were root canal treated and fiber posts were adhesively luted. Specimens were divided into three groups (n = 15) according to the removal technique: long-shaft round bur (EndoTracer #08, Komet, Lemgo, Germany), SonicFlex Endo (KaVo, Biberach, Germany), DT Post Removal Kit (VDW, Munich, Germany). Roots were scanned before post cementation and after post removal using micro-computed tomography. Dentin loss, residual luting material, working time, and the induction of microcracks were assessed. Statistical analysis was performed by using multiple contrast tests (max-t tests, α = 0.05). Correlations between parameters dentin loss/new microcracks and dentin loss/residual material were calculated using Kendall’s tau. Results Post removal with SonicFlex Endo resulted in the highest amount of removed dentin with significant differences to the round bur and the DT Post Removal Kit. No technique was found to completely remove the post and luting material. All techniques induced microcracks with the DT Post Removal Kit presenting the highest number of new defects. No correlation between dentin loss and new microcracks was observed. Deviations from the original root canal occurred in all groups, but no perforation was observed. Conclusions All techniques resulted in dentin loss, residual luting material, and the formation of microcracks. However, no correlation between dentin loss and the induction of microcracks was observed. Clinical relevance As all techniques resulted in microcrack formation and dentin loss, this study emphasizes the risk of iatrogenic damage due to post removal procedures.

La restauración de dientes tratados endodónticamente (RDTE) es una temática en constante estudio, con la finalidad de que se obtengan protocolos clínicos seguros, que puedan mejorar la longevidad de los tratamientos. En los últimos años es creciente la búsqueda por metodologías complementarias a la resistencia de unión, para la evaluación de las RDTE. No obstante, es importante conocer las ventajas y desventajas de cada uno de los métodos complementarios más empleados para evaluar las RDTEs, permitiendo ampliar los conocimientos obtenidos en una investigación. El objetivo de este estudio fue presentar los diferentes métodos utilizados como complementarios a las pruebas de resistencia de la unión y resistencia a la fractura tras una revisión narrativa de la literatura. Se realizó una búsqueda en las bases de datos electrónicas PUBMED, BBO (Biblioteca Dental Brasileña), LILACS (Literatura de Salud de América Latina y el Caribe), SciELO y Medline, utilizando las palabras clave “dientes tratados endodónticamente” y “perno prefabricado”, de 1990 hasta el 2020. De los 360 artículos se excluyeron 295 que no presentaban información relevante para nuestra investigación, donde las metodologías comprendieron pruebas microbiológicas, moleculares, biológicas, metodologías de imagen y métodos de análisis de superficies. Al final de la selección de artículos, se enumeraron las técnicas utilizadas en cada estudio y, a partir de ahí, se realizó una nueva búsqueda bibliográfica, con el objetivo de revisar las técnicas y sus potenciales usos, ventajas y desventajas. Se concluye que las técnicas presentadas son valiosas para incrementar los resultados obtenidos con las pruebas mecánicas en la evaluación de la RDTE con pernos prefabricados.

Various clinical techniques such as removal kits, ultrasonic tips, burs, and drills, have been used for fiber post removal in endodontically treated teeth. In most clinical cases, the dental practitioners prefer to use ultrasonic tips, despite the heat generation and the formation of microcracks induced in the radicular dentin. The purpose of this study was to investigate the effectiveness of using erbium, chromium: yttriumscandium-gallium-garnet (Er,Cr:YSGG) laser (2780 nm) as an alternative fiber post removal technique and to compare it to an ultrasonic method using micro-computed tomography (micro-CT). The operating parameters of the X-ray tube were set to 50 kVp and 300 mA. This approach allowed the generation of 2D lateral projections that were then used to reconstruct the 3D volume in DICOM format. Fiber posts were removed from 20 endodontically treated single-rooted premolars (n = 10) using an ultrasonic vibrator with diamond-coated ultrasonic tip (control method), or Er,Cr: YSGG laser irradiation protocol; average power 2.5 W, repetition rate 20 Hz, pulse duration 140 μs, 40% air and 20% water, and close-contact mode. The number of sections with newly formed microcracks, the loss of dentinal tissue, the amount of the residual resin cement, and the removal time were evaluated for both methods.The data were analyzed using paired t-test, Wilcoxon signed-rank and Mann-Whitney U tests at level of significance a = .05. In the laser-treated group the parameters regarding microcracks formation (21 ± 16) and removal time (4.7 ± 1.1 min) were advantageous compared to the ultrasonic-treated group (42 ± 27 and 9.2 ± 1.0 min, respectively), indicating that Er,Cr:YSGG laser could be an alternative fiber post removal technique.
