Association between Skin-to-Skin Contact Duration after Caesarean Section and Breastfeeding Outcomes

We aimed to explore the association between skin-to-skin contact (SSC) duration after caesarean sections (CSs) and breastfeeding outcomes. A prospective study was conducted in four hospitals in China during January and August 2021. A total of 679 participants with singleton pregnancy who delivered by elective CS after 37 gestational weeks using epidural or spinal anesthesia were included. Logistic regression was applied to assess the association between SSC duration and early initiation of breastfeeding (EIBF), as well as the promoting factors for exclusive breastfeeding (EBF) at hospital discharge. Immediate SSC after CSs was strongly associated with higher rates of EIBF (p < 0.001) and EBF at hospital discharge (p = 0.002). The EIBF rates increased with longer duration of SSC, with the at least 90 min SSC group having the highest EIBF rate (74.5%). Skin-to-skin contact durations of at least 90 min, 60–89 min and 30–59 min were significantly associated with 8.53 times (OR = 8.53, 95%CI: 4.94–14.72, Padj < 0.001), 8.04 times (95%CI: 4.68–13.80, Padj < 0.001) and 6.28 times (95%CI: 3.75–10.51, Padj < 0.001), respectively, higher EIBF rates compared to those without immediate SSC. After multiple-testing correction, the rates of EBF at hospital discharge were found to be independent of the duration of SSC (Padj = 0.12). Early initiation of breastfeeding was not a significant predictor of EBF. Our results suggested that SSC is important for EIBF in Chinese baby-friendly hospitals. Skin-to-skin contact should be practiced after CS to promote breastfeeding and providing SSC with longer duration is encouraged to obtain the full benefit; if it is not feasible, a minimum of 30 min SSC could achieve improved EIBF and EBF at discharge.

Background: Skin to Skin Contact (SSC) of mothers and babies immediately after giving birth provides many benefits at the beginning of breastfeeding, an effective intervention can increase the competence of breastfeeding babies compared to just getting routine care. This study aims to analyze the primary study of the effect of mother-infant skin to skin contact on the success of the first breastfeeding with a meta-analysis. Subjects and Method:This study is a systematic review and meta-analysis with the following PICO, population: mothers and newborns. Intervention: skin to skin contact. Comparison: Routine care. Outcome: Successful first breastfeeding. The articles used in this study were obtained from three databases, namely Google Scholar, Pubmed, Science Direct and Springerlink. Keywords to search for articles ("skin to skin contact" OR "early initiation breastfeeding" OR "kangaroo mother care" AND "first breastfeeding" OR "duration breastfeeding" AND "Randomized Control Trials" OR "RCT"). The articles included are full text in English and Indonesian with a Randomized Controlled Trial study design from 2011 to 2021 and reports on the Mean and SD in a multivariate analysis. Article selection is done by using PRISMA flow diagram. Articles were analyzed using the Review Manager 5.3 aplikasi application. Results: A total of 10 randomized controlled trials from Iran, India, Pakistan, Egypt, and Thailand were selected for a systematic review and meta-analysis. The data collected showed that skin to skin contact increased the success of the first breastfeeding by 1.05 times compared to conventional method care (SMD= 1.10; 95% CI= 0.63 to 1.58; p<0.001). Conclusion: Skin to skin contact can increase the success of the first breastfeeding.

The task of public health is to encourage the mother to initiate and maintain breastfeeding of her child. The main proponents of early breastfeeding should be pediatricians and gynecologists in maternity hospitals. Early "skin-to-skin" contact means that after birth, in the first ten minutes, the newborn is placed in close contact with the mother. After it is born and cries, it is transferred to the bare chest of the mother, who is comfortably placed in bed with her head elevated. The child is placed in a longitudinal position in relation to her, between and above her breasts. The child's head is turned to the side with mandatory monitoring of breathing, and both are covered with a clean, dry sheet. The basis of the mechanism is hormonal. With this kind of mutual contact, oxytocin is released in the mother, and catecholamine concentrations increase in the newborn. The first two hours after birth, the newborn is awake, active, sensitive to touch, smell and temperature simulations that he receives from the mother, which is why this period is the best for starting breastfeeding. The advantages of this method for the mother are: easier and faster stopping of bleeding after childbirth, earlier expulsion of the placenta, greater calmness and relaxation, as well as greater self-efficacy in breastfeeding. The advantages for the newborn are: reduction of postpartum stress and anxiety, less crying, better control of reaching optimal body temperature, importance in establishing microcolonization of the intestinal flora and more effective breastfeeding with earlier discharge from the hospital to home. The application of this method is simple and economically profitable, and short-term and long-term effects are achieved for both the mother and the child.
