Comparison of Tc-99m Methylene Diphosphonate, Tc-99m Human Immune Globulin, and Tc-99m–Labeled White Blood Cell Scintigraphy in the Diabetic Foot

Abstract: These results show that four-phase bone scintigraphy with early Tc-99m WBC scanning is preferred for detecting osteomyelitis of the diabetic foot. To evaluate the response to therapy, Tc-99m WBC scans are the preferred method, but if this is not available, Tc-99m HIG scanning can be used.

“…MRI may also provide anatomic information about the presence of a sinus tract, abscess, or muscle involvement [90][91][92][93]. Nuclear medicine scans (especially those with labeled leukocytes and, sometimes, of bone) are highly sensitive and may be useful in some cases [94][95][96][97] but are generally less specific than MRI. 7 Blood cultures should be performed for a patient with a severe infection, especially if the patient is systemically ill (C-III).…”

“…Skeletal scanning can sensitively be applied for the determination of osteomyelitis but fails to differentiate diabetic osteomyelitis from other osteoarthropathic conditions because of higher bone turnover in both (16). There have been extensive reports about the usefulness of 99m Tc-IgG scintigraphy in detecting a variety of infections, including those of the chest, abdomen, hip, and bone; however, as we also observed, this procedure might not be able to discriminate infection from aseptic inflammation (2,3,17,18). 111 In-IgG scintigraphy differentiated infectious from sterile inflammatory processes in 25 adult patients with complicated osteomyelitis or septic arthritis with a sensitivity of 90% and specificity of 95% (19).…”

“…Dört fazlı kemik/işaretli lökosit sintigrafisinin avantajına baktığımızda; osteoartropati ile birlikte bulunan osteomiyelitin tanısında sintigrafinin doğruluğunun %92'ye yükseldiğini ve 4 fazlı kemik/ işaretli lökosit sintigrafisinin özgüllüğünün MRG'den daha yüksek olduğunu görüyoruz (35).…”
