Comparison of Snellen and Early Treatment Diabetic Retinopathy Study charts using a computer simulation

Abstract: A comparison of true (assigned) VA to measured VA, demonstrated, on average, better accuracy and reproducibility of the ETDRS chart, but at the penalty of significantly longer test duration. These differences were most pronounced in the low VA range. The reproducibility using a simulation approach is in line with reproducibility values found in several clinical studies.

“…Unlike microperimetry, which uses decibels, visual acuity has no standardised measurement as distances can be measured in feet or metres. In addition, there are several different methods of visual acuity examination in clinical use [ 6 , 31 , 32 ], making it difficult to directly compare results from different clinics and research centres. Visual acuity measures a person’s ability to discriminate between stimuli when presented on a highly contrasted background [ 6 ].…”

“…Previous studies recommend the ETDRS method of visual acuity measurement in patients with AMD because it has better accuracy and reproducibility than Snellen, particularly in patients with advanced disease [ 6 , 31 , 33 ]. This is especially relevant as visual acuity may not be affected in patients with AMD until the disease has progressed into the late stage [ 3 , 9 , 12 ].…”

“…Impairment of the optic nerve was assessed by VA in each eye separately using ETDRS logMAR charts presented with standard illumination [ 20 ]. Visual acuity was performed at 100 and 5% contrast at screening, baseline, and every 3 months.…”

“…Patients with optic nerve injury represent a good target group to evaluate the efficacy of drugs aimed at restoring neurological function in patients with MS, as the optic nerves are accessible to precise and quantitative measures. Visual function can readily be assessed using ETDRS (Early Treatment Diabetic Retinopathy Study) logarithm of the minimum angle of resolution (logMAR) charts at high and low contrast to determine VA [ 20 ] and automated perimetry to examine visual field defects [ 21 ]. Optic nerve function can be assessed by recording visual evoked potentials (VEPs) [ 22 ] and the pathological changes seen in the thickness of the retinal nerve fiber layer (RNFL) can be accurately quantified via optical coherence tomography (OCT).…”

“…Visual acuity testing with the ETDRS chart generally requires a 4-meter distance and its use may be limited by its relatively high cost, large chart size, poor availability and applicability. [8][9][10][11] Consequently, other methods of visual acuity measurement are still necessary in general ophthalmic offices (eg Snellen chart and Rosenbaum near vision card).…”
