Introduction: An alternative technique for kidney transplantation is presented for patients in whom the use of pelvic vessels is precluded. Patients and Methods: Of 482 cases of kidney or pancreas-kidney transplants, 4 were unsuited to heterotopic grafting, 1 due to multiple operations in the fossae and the 3 others due to extensive vascular occlusive disease. The patients were studied preoperatively by magnetic resonance angiography, which revealed extensive occlusive disease of the distal aorta and/or iliac vessels. Results: The patients received kidneys from living related donors as a third or second transplant opportunity in the orthotopic position (2 cases). Gross dissection of the aorta revealed a thickened wall, impeding direct anastomoses between the recipient’s arteries and the donated organ. After a termino-lateral Dacron graft to the aorta, all cases were stented for uretero-uretero anastomosis. Recovery with respect to the renal graft was uneventful for all cases. Conclusions: Alternative direct grafting from the aorta represents a viable and easy way to perform kidney transplants in patients unsuited to the heterotopic approach.

Kratak sadr`aj: Kod pacijenata sa hroni~nom bubre` -nom insuficijencijom (CRI) na hemodijalizi razvijaju se lipoproteinske abnormalnosti koje mogu doprineti pove}anom riziku za aterosklerozu. Cilj ove studije bio je da se ispita aterogeni rizik kod pacijenata sa hroni~nom bubre`nom insuficijencijom i pacijenata le~enih dijalizom (DTP) merenjem ukupnog holesterola (TC), triglicerida (TG), HDLholesterola (HDL-C), LDL-holesterola (LDL-C) i izra~una -vanjem odnosa faktora rizika: TC/HDL-C, LDL-C/HDL-C. Grupu ispitanika ~inilo je 18 pacijenata sa hroni~nom bubre` nom insuficijencijom i 60 pacijenata na hemodijalizi. Rezultati su upore|eni sa kontrolnom grupom od 85 dobrovoljnih davalaca krvi. Lipidni parametri u serumu ispitani su standardnim metodama. Svi lipidni parametri kod paci je nata na hemodijalizi statisti~ki su se razlikovali u po re |enju sa kontrolnom grupom (p<0,05), dok su pacijenti sa hroni~nom bubre`nom insuficijencijom pokazali zna~ajne razlike samo u trigliceridima i HDL-holesterolu. Hipertrigliceridemija bila je prisutna u obe ispitane grupe pacijenata a HDL-holesterol bio je ni`i u okviru obe grupe. Svi izra~unati aterogeni odnosi bili su vi{i za pacijente nego za kontrolnu grupu. Upore|eni su lipidni parametri kod pacijenata sa hroni~nom bubre`nom insuficijencijom i paci jenata na hemodijalizi, ali statisti~ki zna~ajna razlika je dobijena samo za HDL-holesterol (p<0,05). Povi{ene vrednosti triglicerida i ni`i HDL-holesterol kod pacijenata sa hro ni~nom bubre`nom insuficijencijom doprinose visokoj incidenci kardiovaskularnih bolesti. Kod pacijenata sa hroni~nom renalnom insuficijencijom o{te}en je reverzni transport holesterola od perifernih }elija do lipoproteina, sni`eni su nivoi HDL-holesterola, prisutna je hipertriglice ridemija uz prevalencu malih gustih LDL i povi{eni su nivoi potencijalno aterogenih ostataka (remnant ~estica).Klju~ne re~i: hroni~no renalno oboljenje, hemodijaliza, lipoprotein Summary: Patients with chronic renal insufficiency (CRI) on hemodialysis develop lipoprotein abnormalities that may contri bute to increased risk for atherosclerosis. Phospholipids in high density lipoproteins (HDL) is being used as a negative risk indicator of atherosclerosis. Phospholipids in HDL may not demonstrate the actual level of HDLphospholipids when determined by the precipitation or ultracentrifugal methods, because HDL fractions contain very high density lipoproteins (VHDL) and albumin. In the present study, the true level of phospholipids in HDL was estimated using high performance liquid chromatography (HPLC), and it was compared with the level of phospholipids in HDL determined by the precipitation method. Sera from 18 healthy subjects were used as materials. In the HPLC method, the HDL fraction was extracted making sure that it contained no free albumin, which is albumin not bound to phospholipids. The HDL fraction was separated into subfractions. It was found that phospholipids in the VHDL fraction make a 20.2 + 7.3% (mean± S.D.) part of the total HDL-phospholipids. A large part of the VHDL fraction was constituted of albumin-bound phospholipids. A significant correlation was observed between HDL-phospholipids determined by the precipitation method, which contain albumin, and the actual HDL fraction phospholipids determined by HPLC, which do not contain VHDL (r = 0.903, p<0.01). These results suggest that HDL-phospholipids values determined by the precipitation method give useful clinical data.
