Aim : We designed a cross-sectional study in order to determine the prevalence of inguinal hernia and penoscrotal abnormalities in the Lorestan province, situated in the west of Iran. Methods : We studied 3205 elementary-school boys, aged 6-12 years, who underwent clinical examinations of the groin and genitalia in 2002.Results : Abnormalities were detected in 213 children (6.64%). The most frequent anomaly was indirect inguinal hernia, seen in 78 children (2.4%). The other abnormalities were retractile testes in 39 boys (1.22%), undescended testes in 36 boys (1.12%), hydrocele in 28 boys (0.87%) and hypospadiasis in 25 boys (0.78%). Also, three children had micropenises, two had epispadiasis and another two boys had varicoceles. Ambiguous genitalia and apenia were not seen in the present series. Most of the parents were not aware of their children's anomalies (60.1%). Conclusions : Education of the public and medical staff about these abnormalities and screening system are needed to improve the outcome.

A 12-year-old boy with a left-sided varicocele (Grade 3) was assessed preoperatively and followed postoperatively by computer-assisted infrared thermography. The temperature of the left scrotum was 35.4 degrees C before surgery which was 4 degrees C higher than that of the right scrotum. After ligation of the left internal spermatic vein and removal of the varicocele, scrotal thermograms 39 days and 12 months after operation were normal. Therefore, it seems that thermography is very valuable in the evaluation of testicular function in children with varicocele, in whom semen analysis is impossible.

The aim of this prospective study was to find out the incidence of groin hernias and external genitalia abnormalities in children in the City of Karak at the south of Jordan for referral and early treatment, as well as to educate the population about the risk and complication of these abnormalities.A total of 2 038 male children aged 6-12 years, primary school attendants, underwent careful clinical examination of groin, penis and scrotum. Abnormal findings were detected in 381 male children (18,7%).The abnormalities were as follows: indirect inguinal hernia in 280 children, undecided testicles in 44 children, retractile testicles in 26, hypospadias in 15, varicocele in 10, hydrocele in five children. Herniotomy was detected in 66 with 4 cases failures, orchidopexy in five children with two failures, hypospadia in one child who failed. No one with hydrocele or varicocele underwent surgery.We conclude that indirect inguinal hernia is common in children. Undescended testicles are not uncommon. The majority of children with abnormalities are late for treatment. Failure rate of treatment is high. Education of population is needed to improve the outcome.
