INTRODUCTIONDay care laparoscopic surgery is one of the most common surgical procedures performed and rapidly increasing nowadays. General anaesthesia is routinely provided by use of an intravenous sedative-hypnotic as an induction agent followed by inhalational agents for maintenance of anaesthesia. Widespread availability of non-pungent and rapidly acting volatile anaesthetic agents are in increasing use for induction and maintenance of general anaesthesia in these patients and are widely used in day-care surgeries. However, early recovery and postoperative nausea vomiting (PONV) depends on the anaesthetic technique being used.1,2 The use of appropriate anaesthetic agents that provide fast and smooth induction, allow fast changes in intensity while ABSTRACT Background: Day care laparoscopic surgical procedures are rapidly increasing nowadays. Rapid emergence and early recovery from anaesthesia with minimal complications are desired. Both propofol and sevoflurane meet above criteria and established as agents of choice in laparoscopic surgeries for induction and maintenance of anaesthesia. So this study aimed to compare sevoflurane with propofol for intraoperative haemodynamic changes with postoperative recovery profile in patient's undergone laparoscopic cholecystectomies under general anaesthesia. Methods: In this prospective randomized study, sixty patients of either sex, 18-60 years with ASA grade 1 and 2 scheduled for laparoscopic cholecystectomies under general anaesthesia were randomly allocated into two groups. In Group S, patients were maintained on sevoflurane anaesthesia (0.5-2.5%) while in Group P, patients were maintained with propofol infusion (75-125 µg/kg/min) along with O 2 (50%) and N 2 O (50%).The intraoperative haemodynamic parameters, recovery characteristics and postoperative nausea and vomiting (PONV) were observed in both groups. Results: The mean baseline haemodynamic parameters (HR, SBP, DBP, MBP, SpO 2 and EtCO 2 ) were comparable in both groups, (P>0.05). No significant difference in HR was at observed any time interval, P>0.05, however, SBP, DBP and MBP were significantly lower in propofol group at different time intervals, P<0.05, but clinically not significant and patients remained haemodynamically stable in both groups. The mean time for all recovery characteristics were significantly shorter in sevoflurane group as compared to propofol group, (P<0.01). However the incidence of PONV was significantly more in sevoflurane group. Conclusions: Sevoflurane can be used as an effective alternative to propofol for maintenance of anaesthesia in day care laparoscopic procedures as it has better recovery profile with stable haemodynamic parameters.

Background: Some studies have reported the effects of anesthesia induction using a single propofol dose and low ketamine doses in short-term outpatient operations. Objectives: In this study, we aimed to evaluate the intra and post-operative hemodynamic effects of ketamine-propofol mixture (Ketofol) infusion in comparison with propofol infusion. Methods: This study was performed on 54 class I and II of the American Society of Anesthesia patients aged 15 to 45 years who were candidates for leg fracture surgery. The patients were randomly assigned to propofol and ketofol groups. In the propofol and ketofol groups, propofol infusion (100 µg/kg/min) and propofol-ketamine infusion (50 µg/kg/min propofol + 25 µg/kg/min ketamine) were used for the maintenance of anesthesia, respectively. Heart rate and systolic, diastolic and mean blood pressure before, immediately after the induction of anesthesia and at 10-minute intervals were measured and recorded. Pain, nausea, and vomiting were recorded immediately after surgery and each 2 hours until 6 hours. Results: Systolic, diastolic and mean blood pressure were significantly higher in the ketofol group than in the propofol group at 10 -60 min intervals (P < 0.05). There was no significant difference, however, between the two groups in terms of the severity of nausea and pain and vomiting frequency. Conclusions: Infusion of hypnotic doses of ketofol leads to increase in diastolic and systolic blood pressure and improves blood pressure stability in addition to inducing more as compared with propofol infusion, but it leads to higher risk of nausea and vomiting.

Praktik anestesi membutuhkan kualitas anestesi yang baik, efek samping minimal, serta pemulihan cepat. Propofol dan sevofluran memiliki efek farmakologis pemulihan pascaanestesi yang cepat. Penelitian ini bertujuan mengetahui perbandingan sevofluran dengan propofol target controlled infusion (TCI) sebagai rumatan anestesi terhadap waktu pulih sadar dan komplikasi mual muntah pada operasi timpanoplasti. Penelitian dilakukan periode Agustus–November 2018 di RSUP Dr. Hasan Sadikin Bandung. Penelitian merupakan prospektif eksperimental menggunakan uji klinis acak buta tunggal terhadap 44 subjek yang dibagi acak menjadi dua kelompok, yaitu kelompok propofol TCI (kelompok P, n=22) dan kelompok sevofluran (kelompok S, n=22). Selama operasi dicatat lama operasi, jumlah opioid, waktu pulih sadar, dan komplikasi mual muntah. Analisis statistik data numerik dengan uji t tidak berpasangan dan Mann Whitney, data kategorik dengan uji chi square. Hasil penelitian tidak ada perbedaan signifikan (p>0,05) untuk karakteristik pasien, lama operasi, dan jumlah opioid perioperatif. Waktu pulih sadar kelompok sevofluran lebih cepat signifikan dibanding dengan propofol TCI (10,26±1,91 menit dan 13,36±1,72 menit; p<0,05). Komplikasi mual muntah pascaoperasi kelompok sevofluran lebih besar signifikan dibanding dengan propofol TCI (15 dan 1; p<0,05). Simpulan, sevofluran memiliki waktu pulih sadar lebih cepat dibanding dengan propofol TCI, namun komplikasi mual muntah sevofluran lebih besar. Comparison between Effect of Sevoflurane with Propofol Target Controlled Infusion (TCI) as Maintenance of Anesthesia on Emergence Time and Nausea-Vomiting Complication in Timpanoplasty SurgeryCurrent modern anesthesia practice requires good anesthetic quality, minimum side effect, and early recovery. Propofol and sevoflurane have pharmacological properties that include fast recovery after anesthesia. The aim of this study was to evaluate the comparative effects of sevoflurane versus propofol target controlled infusion (TCI) for maintenance of anesthesia with respect to emergence time and postoperative nausea and vomiting (PONV) complication in patients undergoing timpanoplasty surgery. The study was carried out from August–November 2018 in Dr. Hasan Sadikin Bandung Hospital. It was a prospective single blind experimental study conducted on 44 subjects who were randomly divided to receive either propofol TCI (group P, n=22) or sevoflurane (group S, n=22). Duration of surgery, total ammount of opioid used, emergence time, and postoperative nausea and vomiting complication were recorded. Numerical data were tested by unpaired t test and Mann Whitney while categorical data were tested by chi-square. The results showed that patient characteristics, duration of surgery, and total amount of opioid used were not significantly different (p>0.05). Emergence time of the sevoflurane (S) group was significantly faster than the propofol (P) group (10.26+1.91 and 13.36+1.72, respectively; p<0.05). The complication of postoperative nausea and vomiting in group S was significantly higher compared to group P (15 and 1, respectively; p<0.05). It is concluded that sevoflurane has faster emergence time compared to propofol TCI, but PONV complication of sevoflurane is higher.
