Comparison of Pure-Tone Audiometry Analysis in Sudden Hearing Loss Studies

Abstract: The lack of an acceptable agreement between most measurement methods assessed in this report prevents a meaningful comparison of published clinical studies on ISSHL. For comparison of efficacy between different treatments in ISSHL and for the planning of controlled clinical trials, it is necessary to agree on standardized outcome measures, including the frequencies averaged and the criteria of success.

“…Commercially available steroid preparations contain preservatives, such as alcohol, and consequently the presence and concentration of these must be considered when choosing a preparation for clinical use in the cochlea. The presence of preservatives may also influence the rate of absorption across the round window, with alcohol having been shown to increase the rate of diffusion and cochlear uptake [71]. The rates of infection from intravitreal steroid injection are similar to those observed with other injections, being approximately 1% for the anti-vascular epithelial growth factor monoclonal antibody ranibizumab [72].…”

“…Consequently, we cannot rule out an influence of these treatments on ISSHL outcome. To calculate the recovery rate, contralateral ear was used as baseline in this study, as recommended in a recent meta-analysis [Plontke et al, 2007]. Rather than the hearing improvement, the severity of the hearing loss on the last audiogram is important because it has a major impact on the possibility of hearing rehabilitation and on binaurality.…”

“…The number of applications from single injection to four injections over several days has been reported. The diversity of CS use, concentration, and the number of applications contribute to the difficult comparison of CS treatments [18] . Studies use various measurements of hearing and criteria of improvement that add to the controversy of best practice [18] .…”

Studies use various measurements of hearing and criteria of improvement that add to the controversy of best practice [18] . A larger randomized controlled trial investigating the efficacy of the IT DEX would be more appropriate; however, as the Slovene National ENT Association Assembly recommended IT DEX as a standard therapy, we were not able to perform it [13] .…”
