Comparison of pulmonary segmentectomy and lobectomy: Safety results of a randomized trial

Abstract: Background: No definitive comparisons of surgical morbidity between segmentectomy and lobectomy for non-small cell lung cancer have been reported. Methods: We conducted a randomized controlled trial to confirm the noninferiority of segmentectomy to lobectomy in regard to prognosis (trial No. JCOG0802/ WJOG4607L). Patients with invasive peripheral non-small cell lung cancer tumor of a diameter 2 cm were randomized to undergo either lobectomy or segmentectomy. The primary end point was overall survival. Here, we … Show more

“…Segmentectomy was categorized into simple or complex from the standpoint of technical aspect: resection of superior segment of the lower lobe or the lingular segment was de ned as simple. Complex segmentectomy was resection of segment other than that to be resected in the simple segmentectomy, beside, complex segmentectomy was de ned as resection of a segment that had more than one intersegmental plane (7).…”

“…In addition to the increasing frequency of sublobar resection of GGO-dominant tumors, studies have been conducted to demonstrate the e cacy of sublobar resection for solid-dominant tumors of 2 cm or less. Two important randomized controlled trials (JCOG0802 and CALGB140503) of sublobar resection for solid-dominant tumors are being conducted and will produce results in a few years [6][7][8]. The results of these studies will clearly demonstrate the e cacy of sublobar resection in small solid-dominant lung cancer.…”

“…Although the optimal surgical treatment for lung cancer has long been lobectomy, segmentectomy may become a standard treatment for early-stage lung cancer. 1) Previous studies have shown that segmentectomy is oncologically comparable with lobectomy for early-stage lung cancer. 2) Segmentectomy also has the advantage of preservation of lung function.…”
