Background: Extensive studies are conducted on heart, liver and endocrine abnormalities in thalassemia owing to their direct effect on survival, however, lung dysfunction has never been focused upon and is one of the least understood complications in β thalassemia. There's a vacuum for data on pulmonary function tests in β thalassemia major in literature from India. Authors aimed to study pulmonary function and type of abnormality in cases with β thalassemia major above the age of 8yrs and to correlate the result with age and serum ferritin levels. Methods: Demographic data, hemoglobin value, serum ferritin levels, chelation details and transfusion requirement were analyzed. Spirometry was performed using COSMED pulmonary function test (PFT). Results: Among the 34 subjects studied, 21 were boys, and 13 were girls. Mean serum ferritin levels of the group was 3610.82±2679.51ng/mL and did not show a significant correlation with age, years of transfusion, and years of chelation. Forced vital capacity (FVC), forced expiratory volume in 1 st second (FEV1) % values were lower in boys when compared to girls. PFT showed a restrictive pattern in the study group (FEV1/FVC=>0.7) with significant involvement in 73.5% of cases (FEV1<80%). A statistically significant negative correlation was observed between age and FEV1% (r=-0.577, p=<0.01) highlighting the importance of duration of iron overload. However, there was no significant linear correlation between restrictive lung disease and serum ferritin level (r=-0.06, p=0.75). Conclusions: Restrictive pattern was the most common abnormality, and it did not correlate with serum ferritin. Pulmonary function monitoring would help in identifying children with significant morbidity and help in initiating an early intervention to improve the quality of life.

Kebiasaan beraktivitas sehari-hari membuat anak jalanan sering terpapar dengan polutan udara di jalan dan juga kebiasaan hidup yang tidak sehat seperti merokok membuat anak jalanan berisiko mengalami penurunan fungsi paru. Untuk mendapatkan gambaran nilai faal paru anak jalanan sebagai kelompok yang berisiko tinggi akibat pencemaran udara dan faktor-faktor yang mempengaruhinya. Metoda. Metoda. Metoda. Metoda. Metoda. Desain cross sectional terhadap kelompok anak jalanan di wilayah Jakarta Pusat yang dibina di empat rumah singgah. Pada subyek penelitian yang memenuhi kriteria inklusi dilakukan pendataan riwayat pribadi dan penyakit, pemeriksaan fisik serta pengukuran nilai arus puncak ekspirasi (APE). Subyek dipilih secara stratified random sampling dengan besar sampel ditentukan secara proporsional dan penghitungan untuk mencari koefisien korelasi. Hasil. Hasil. Hasil. Hasil. Hasil. Telah dilakukan pemeriksaan dan pengukuran nilai APE dengan alat mini wright peak flowmeter (MWPFM) terhadap 99 anak jalanan yang terdiri dari 61 anak laki-laki dan 38 anak perempuan. Rentang usia terbanyak laki-laki 13-15 tahun, perempuan 7-12 tahun, 73 masih bersekolah dan 26 anak putus sekolah. Dengan kuesioner international study of asthma allergic in childhood (ISAAC) didapatkan 2 anak dengan riwayat pernah menderita asma. Status gizi anak jalanan menunjukkan 69 anak (70%) dengan gizi baik, 27 anak ( 27%) dengan gizi kurang, dan 3 anak (3%) bergizi lebih. Kebiasaan merokok pada anak jalanan banyak ditemukan pada kelompok anak laki-laki, yaitu pada kelompok usia 13-15 tahun 9 orang (26%), 16-18 tahun 4 orang (50%) dan 1 orang anak perempuan usia 17 tahun. Kelompok usia terbanyak menunjukkan kelompok usia sekolah tingkat SMP dan SMA. Kesimpulan. Kesimpulan. Kesimpulan. Kesimpulan. Kesimpulan. Nilai APE pada kelompok anak jalanan dipengaruhi oleh beberapa faktor antara lain jenis kelamin, umur, tinggi badan, berat badan dan luas pemukaan tubuh. Nilai APE pada anak jalanan laki-laki sebagian besar berada di bawah nilai rata-rata menurut nomogram Godfrey; pada anak perempuan relatif sama baik untuk nilai di atas rata-rata maupun di bawah rata-rata. Nilai APE pada anak jalanan tidak jelas berhubungan dengan lama paparan, namun nilai indeks APE pada kelompok dengan paparan lebih dari 5 tahun dan terpapar lebih dari 5 jam sehari cenderung lebih rendah dibandingkan kelompok anak jalanan yang lain. Nilai APE anak jalanan yang merokok tidak lebih rendah dari pada nilai tersebut pada anak jalanan yang tidak merokok.Kata kunci: uji fungsi paru, anak jalanan, nilai arus puncak ekspirasi.

Prevalence of pulmonary dysfunction in patients with beta thalassemia major: a systematic review and meta-analysis

Background Many studies have been conducted on heart, liver, and endocrine abnormalities in thalassemia; however, studies on pulmonary dysfunction (PD) have been limited. Previous studies on the prevalence of restrictive lung disease (RLD) and obstructive lung disease (OLD) in β-thalassemia major patients have lacked agreement. Objective To assess the prevalence of PD in β-thalassemia major patients by systematic review of the literature and meta-analysis. Methods We searched Cochrane library, PubMed, Web of Science, MEDLINE, Scopus, and Embase for relevant articles. Articles were selected according to the inclusion criteria and data were extracted. The primary outcome was prevalence of pulmonary dysfunction in β-thalassemia major with 95% confidence interval (95%CI). Subgroup analyses were applied to explore the prevalence in different age groups, regions, and serum ferritin levels. Sensitivity analysis and publication bias assessment were also conducted. Results A total of 37 studies comprising 1,467 cases were included in this analysis. Pulmonary dysfunction was present in 64.7% (95%CI 57.6 to 71.1) of cases. The pooled prevalence of RLD (44.9%) was higher than that of OLD (7.6%) and diffusion impairment (DI) (35.6%). Subgroup analysis revealed that the region with the highest pooled prevalence of PD was the Americas (75.2%). The highest prevalence of RLD and DI was found in Asia (48.2% and 44.6%, respectively) and that of OLD in Europe (9.7%). Sensitivity analysis showed that the pooled results were robust. Conclusion A high prevalence of pulmonary dysfunction, mainly RLD rather than OLD, was detected in β-thalassemia major patients.
