ÖzetAmaç: Amacımız yeni endoskopi ünitesi kurulmuş olan bir devlet hastanesinde üst gastrointestinal endoskopi yapılmış olan hastaların özelliklerini belirlemektir. Hastanemiz bölgesindeki gastrointestinal hastalıklarının spektrumunu ortaya koymak istedik. Gereç ve Yöntem: Üst gastrointestinal endoskopi yapılan hastaları endoskopi sonuçları, yaş, cinsiyet, şikâyetleri, klinik özel-likleri, anestezi tipi, biyopsi gerekliliğine göre analiz ettik. 2013 Aralık-2014 Temmuz arasında 256 hastayı değerlendirdik. Tüm endoskopiler aynı cerrah tarafından yapılmıştır. Bulgular: En fazla karşılaşılan şikâyet epigastrik ağ-rıydı (n=112, %43. 8). Diğer şikayetler dispepsi (n=84, %32.8), retrosternal yanma (n=42 %16.4) , bulantı (n=4, %1.6) , kusma (n=2, %0.8), disfaji (n=6, %2.3) olarak belirlendi. Hastalarda 218 gastrit (%85.2), 120 özefa-jit(%46.9),76 duedonit(%29.7), 64 hiatal herni(%25), 18 duedonal ülser(%7), 4 gastrik ülser(%1.6), 20 alkalen safra reflüsü(%7.8), 26 Gastroözefagial reflü (GÖR), (%10.2) tespit ettik. 10 hasta normal olarak değerlendirildi. Hastaların 186'sından biyopsi alındı. Tartışma: Endoskopinin erişilebilirliğinin arttı-rılması ile bir erken tanı tetkiki haline gelebilir. Ayrıca alarm semptomları göz ardı edilmemeli ve semptomatik olan hastalarda derhal endoskopi yapılmalı-dır. Bu çalışmada yapılmış olan üst gastrointestinal sistem endoskopilerinin bir sonucu olarak gastrit, özefajit, duodenit ve hiatal herni bizim bölgemizde sık karşılaşılan gastrointestinal hastalıklardır. We want to present the spectrum of gastrointestinal diseases in our hospitals region. Material and Method: We analyzed patients upper endoscopy results according to age, sex, complaints, clinical characteristics, type of anesthesia, and the necessity of biopsy. We reviewed 256 patients data between 2013 December-2014 July. All endoscopies were performed by same surgeon. Results: The highest complaint was epigastric pain (n=112, 43, 8%). Other complaints were followed as dyspepsia (n=84, 32.8%), heartburn (n=42, 16.4%), nausea (n=4, 1.6%), vomiting (n=2, 0.8%), dysphagia (n=6, 2.3%). We determined 218 gastritis (85.2%), 64 hiatal hernia (25%), 120 esophagitis (46.9%), 76 duodenitis (29.7%), 4 gastric ulcer (1.6%), 18 duodenal ulcers (7%), 20 bile reflux (7.8%), 26 Gastro esophageal reflux disease (GERD) in patients (10.2%). 10 patients reported as normal (3.9%). Biopsy was performed in 186 of the patients. Discussion: Endoscopy can become an early diagnostic examination by increasing the availability of endoscopy. Also alarm symptoms should not be ignored and endoscopy should perform immediately in symptomatic patients. The correlation between low serum PG level and H. pylori infection with the development of gastric cancer has caused considerable concerns all over the world. Some authors exclaimed that gastric cancer developed only in patients infected with H. pylori, whereas the other had different findings. In this study, 1,501 adult local residents with determined serum PG levels and anti H. pylori IgG status were followed for 14 years for the development of gastric cancer in a rural community with high risk of gastric cancer in Hebei Province, China. The results showed the accumulated gastric cancer incidence in the subjects with abnormal PG level and those with H. pylori infection were all significantly higher than that in the corresponding normal controls (53.9% vs. 12.7%, p < 0.05 and 23.1% vs. 5.93%, p < 0.05). The highest gastric cancer incidence was seen in the subjects with both abnormal serum PG and positive H. pylori (56.0%), and followed by the subjects with abnormal PG and negative H. pylori (47.6%) and those with normal serum PG and positive H. pylori (18.4%). The abnormal serum PG level (OR 3.029) and H. pylori infection (OR 4.345) were all risk factors for the development of gastric cancer. The results suggested that the subjects with abnormal serum PG level and/or positive H. pylori infection in the rural area of China were all high risk population for gastric carcinoma and the subjects with both abnormal serum PG and positive H. pylori infection were at especially high risk for the development of gastric carcinoma.Low level of serum pepsinogen I (PGI) and/or the ratio of PGI to PGII is an accurate indicator of chronic atrophic gastritis, which is a very important precursor of gastric cancer. 1,2 The test for serum PG has been incorporated into gastric cancer screening programs to identify people at high risk for gastric cancer in many countries since 1990s. [3][4][5][6][7] As the pepsinogen test method is virtually used as a prescreening test for high risk subjects rather than to find gastric cancer itself, 8,9 the correlation between serum PG abnormality and the development of gastric cancer has caused considerable concerns all over the world.Despite the obvious decline in gastric cancer incidence in Western countries in the past a few decades, gastric cancer remains the leading cause of cancer death in China. 10 Gastric cancer screening program using serum PG test has not been widely used in China. The purpose of the present study is to investigate the effects of hypochlorhydria induced by proton pump inhibitor (PPI) administration and intake of a dairy product fermented by lactobacilli (DFL) on bone metabolism in growing rats. Male rats, aged 3 weeks, were divided into two groups: a control group fed a casein-based diet and a group fed a DFL-based diet. Each group was fed its respective experimental diets for 9 d. At day 5 of the feeding period, each group was divided into two subgroups: one that received a saline injection and one that received a PPI injection. Rats were subcutaneously administered saline or PPI for 5 d. Faecal Ca excretion was determined from day 6 to day 9. At the end of the experiment, plasma and femurs were collected. Administration of PPI significantly decreased bone mineral density (shown by X-ray computerised tomography) and bone strength (shown by a three-point bending test) in the control group. Plasma osteocalcin, type I collagen C-telopeptides, 1,25-dihydroxyvitamin D and parathyroid hormone concentrations were elevated by PPI administration in the control group. Faecal Ca excretion and urinary P excretion in the control group were remarkably increased by PPI administration. On the other hand, these adverse effects of PPI were not observed in the DFL group. These results suggest that hypochlorhydria-induced bone loss may result from high bone turnover induced by secondary hyperparathyroidism due to Ca malabsorption and that DFL intake cancels these adverse effects probably via improving Ca malabsorption in growing rats.Key words: Hypochlorhydria: Fermented milk: Bone: Calcium Most physiological functions decline progressively with age. Recently, Aoki et al. reported that chronic atrophic gastritis, a factor of hypochlorhydria, is very common among the elderly (4) .There are some studies suggesting the association between hypochlorhydria and bone loss. Many researchers have reported that gastrectomy induces bone loss in rats (5 -8) and human subjects (9 -12) . However, gastrectomy in itself has multiple risk factors other than hypochlorhydria for bone loss. For example, the production of the stomach hormone ghrelin, which is involved in bone physiology, is suppressed by gastrectomy (13) . Some studies have shown that the use of proton pump inhibitors (PPI), the most potent acid-suppressing drugs, is associated with an increased risk of hip fracture (14,15) . However, there is limited research showing the effects of hypochlorhydria induced by PPI on bone metabolism in human subjects or animals.The absorbability of Ca is dependent on its solubility. The acidity in the stomach and resulting chyme increases Ca solubility (16) .
