Comparison of narrow band imaging and the Storz Professional Image Enhancement System for detection of laryngeal and hypopharyngeal pathologies

Abstract: Both methods, NBI endoscopy and SPIES system, are comparable in detection and analysis of superficial neoangiogenesis, typical for benign lesion and for precancerous or cancerous changes in larynx and hypopharynx.

“…Indeed, even though they are based on slightly different engineering details, both these techniques aim to enhance the vascular network of mucosal lesions and thus differentiate them according to their biologic potential. As already demonstrated by Stanikova et al, NBI and SPIES appear to provide comparable outcomes with excellent diagnostic agreement between them [11]. Furthermore, coupling biologic endoscopy with contact endoscopes is a promising and inter-rater-reliable technique to further improve the diagnostic accuracy of intraoperative endoscopic evaluation [20,21].…”

“…In fact, many technical innovations and endoscopic tools have been recently developed to better evaluate such lesions in the pre-and intraoperative settings, including white light (WL) videoendoscopy and different kinds of biologic endoscopy, such as narrow band imaging (NBI) and the Storz professional image enhancement system (SPIES) [6,10,11]. These techniques, together with videolaryngostroboscopy (VLS) and saline infusion (SI) into the Reinke's space, help in obtaining detailed information about the tissue characteristics, vascular network, and superficial versus deep involvement of the different layers of the lamina propria by the lesion itself, thus allows for a tentative diagnosis (at least in terms of intraepithelial or more invasive disease) and adequate resection using a one-step excisional biopsy approach [9,12,13].…”

“…One of the recommendations by National Institute for Health and Care Excellence (NICE) 12 is to emphasize the role of NBI endoscopy as their strategy for an earlier tumour detection. As both NBI endoscopy and SPIES endoscopy methods are comparable in recognition and analysis of benign and malignant lesions 9 , SPIES endoscopy should also be recommended as one of the strategy for an earlier diagnosis of UADT tumours. Both NBI and SPIES endoscopy systems form an optimal strategy for preventing the progression of the cancer and improving the prognosis by earlier detection.…”

“…It has been established that both NBI and SPIES endoscopy systems are comparable in recognition and analysis of vascular patterns that is typical for benign and malignant lesions 9 . Thus, any classifications used and valid for NBI should also be applicable for SPIES as well.…”

“…As a matter of fact, a great help in trying to distinguish high-from low-risk lesions (i.e., lesions with a low malignant potential like keratosis without atypia or mild dysplasia) may be derived from the systematic in-office use of bioendoscopic techniques such as the Narrow Band Imaging (NBI) by Olympus or the Storz Professional Image Enhancement System (SPIES), that have been repeatedly demonstrated to be superior to WL endoscopy alone in giving a valuable insight into the biologic behaviour of LDs [4]. As recently demonstrated by Stanikova and coworkers, both methods yield comparable outcomes in distinguishing benign/low-risk from malignant/high-risk lesions, with a sensitivity and specificity of 83% and 98%, respectively, for NBI, and 86% and 96% for SPIES [5]. Both techniques rely on a widely-applied classification of neoangiogenetic vascular changes that, when present, have the aspect, defined by Arens and coworkers, of "perpendicular vessels", indicative of a potentially malignant disease (from mild dysplasia upward) [6].…”
