Comparison of myelin water fraction values in periventricular white matter lesions between multiple sclerosis and neuromyelitis optica spectrum disorder

Abstract: Multiple sclerosis (MS) and neuromyelitis optica spectrum disorder (NMOSD) are inflammatory autoimmune diseases of the central nervous system. We hypothesized that the degree of demyelination within lesions in MS and NMOSD would differ as the pathophysiology of the two diseases do. We used myelin water imaging to compare the myelin water fraction (MWF) in 106 periventricular white matter (PVWM) lesions in 27 MS patients and 51 PVWM lesions in 20 NMOSD patients. The MWF was significantly reduced in the MS compa… Show more

“…Much of the pioneering in vivo work in myelin water imaging has been studies of MS. MS plaques, or lesions, show variably decreased MWF (Figure 7) (38, 58,64,85,93,97,122,166,171,180), averaging approximately half that of Figure 2). normal-appearing white matter (NAWM) (85).…”

“…Then voxel-wise multiecho data were processed to generate a T2 distribution by fitting stimulated-echo corrected multi-exponential functions 7 . The following parameters, which were common for clinical studies [18][19][20][21] , were used for the fitting: number of exponential functions = 120; T2 range = 15 to 2000 ms, logarithmically spaced; and chisquare regularization with the constraint of 1.020 2 ≤ 2 ≤ 1.025 2 . From the T2 distribution, MWF was calculated by dividing the sum of the signals from 15 to 40 ms by the sum of the entire T2 distribution.…”

“…Myelin water fraction (MWF), defined as the fraction of the total signal which has T 2 < 40 milliseconds at 3 Tesla, has been histologically validated as a marker of myelin density in human tissue, 30–32 and has good scan‐rescan reproducibility (coefficient of variation 4% in cerebral white matter) 33 . MWI has been used to investigate myelin in a wide range of diseases including multiple sclerosis, neuromyelitis optica spectrum disorder, schizophrenia, Niemann‐Pick disease, prenatal alcohol exposure, amyotrophic and primary lateral sclerosis, Krabbe disease, autism, stroke, and traumatic brain injury 34–47 . Rapid myelination has been demonstrated in early childhood and adolescence using multicomponent driven equilibrium single pulse observation of T 1 and T 2 (mcDESPOT), an alternate approach to MWI using a steady‐state acquisition 48–53 .…”
