BackgroundNosocomial outbreaks of multidrug-resistant Acinetobacter baumannii are of worldwide concern. Using pulsed-field gel electrophoresis (PFGE), multilocus sequence typing (MLST), and multiple locus variable number tandem repeat sequence (VNTR) analysis (MLVA), the present work examines the genetic diversity of the endemic and epidemic A. baumannii clones isolated in a single hospital over a twelve-year period.ResultsPFGE analysis of 405 A. baumannii-calcoaceticus complex isolates detected 15 A. baumannii endemic/epidemic PFGE types (EE1 to EE15) that grouped into five clusters: EE1-EE8, EE9, EE10, EE11 and EE12-EE15. The MLST sequence type (ST) distributions were: international clone II (ST-2) 60%, international clone III (ST-3) 26.7%, ST-15 6.7%, and ST-80 6.7%. MLVA-8Orsay returned 17 allelic profiles. The large (L) VNTR marker profiles were fully concordant with the detected STs, and concordant with 14 up to 15 PFGE types. Imipenem resistance was detected in five PFGE types; the prevalence of the blaOXA-58-like and blaOXA-40-like genes was 60% and 40% respectively.ConclusionsPFGE proved to be a vital tool for analysis of the temporal and spatial distribution of the clones. MLST and the VNTR L-markers grouped the isolates into clonal clusters. The wide diversity of MLVA small (S)-markers, however, did not permit clustering. The present results demonstrate the persistence of several endemic PFGE types in the hospital, the involvement of some of them in outbreaks, and the inter hospital transmission of extensively drug-resistant ST-15 and ST-80.

Objectives: Acinetobacter baumannii has emerged worldwide as a major nosocomial pathogen. The aim of this study was to evaluate the MLVA-8Orsay scheme suitability for clustering and differentiating A. baumannii strains isolated from two hospitals in Tehran. Methods: A panel of eight variable-number tandem-repeat (VNTR) markers was used for typing analysis (MLVA-8Orsay) of 89 carbapenem-resistant A. baumannii strains isolated from two hospitals in Tehran. Results: Abaum0845 (S-VNTR marker) was excluded from the MLVA scheme because of its great allelic diversity at this locus. The modified MLVA-7 scheme provided excellent discriminatory power and detected 82 MLVA types among the 83 isolates. Additionally, the great diversity associated with MLVA markers did not permit to apply the previously proposed criteria for clustering the isolates. However, six MLVA complexes were defined assuming a 71% genetic similarity cut-off, corresponding to the existence of identically sized alleles for at least five VNTR markers. The blaOXA-23-like gene (93.2%) was the most prevalent gene in resistant isolates collected from the two hospitals. Conclusions: MLVA-7 discriminatory power was found to be excellent. This method was also appeared to be useful and reliable in epidemiological investigations of A. baumannii strains. MLVA-7 analysis revealed that carbapenemase-carrying A. baumannii isolates might be polyclonal and provided evidence for gene transfer among different strains collected from the two hospitals. Overall, the study provided evidence supporting the use of MLVA-7 scheme as an alternative or complementary method for typing of A. baumannii isolates.

Background: Despite a relatively low virulence of Acinetobacter baumannii isolates, emerging multidrug-resistant (MDR) strains to pose a formidable threat to patients, particularly in ill patients in intensive care unit (ICU). Objectives: The aim of the present study was to determine the genetic relatedness and antimicrobial susceptibility patterns in the endemic clones of A. baumannii isolated from patients in the ICU. Methods: Fifty-five non-repetitive A. baumannii isolates were examined for antimicrobial susceptibility, oxacillinase genes, class 1 integrons and genetic relationships by PFGE. Results: Antibiotic susceptibility testing showed that 21.8%) 12 isolates) were resistant to all tested antibiotics. Resistant to carbapenems were up to 85%. OXA-23, OXA-24 and OXA-58 genes were detected in 81.81%, 16.36% and 1.81% isolates respectively. The ISAba1 element upstream of blaOXA-51 was detected in 18 (32%) isolates, and 22 (40%) isolates had an ISAba1 insertion sequence upstream of the blaOXA-23. Integron class 1 was detected in 25 (55.5%) OXA-23 carrying isolates, 2 (22.2%) in OXA-24 positive isolates and in one OXA-58 carrying isolate. PFGE analysis resolved 50 distinct pulsotypes, Most of the isolates were scattered throughout across the dendrogram and a few grouped as clusters. Conclusions: No significant association has been found between the pulsotype of each isolate and MDR patterns and the presence of carbapenemase genes, however, highly resistant blaOXA-23 gene carrying endemic clones of A. baumannii disseminated in the ICU of two hospitals. Therefore, active surveillance and health policies are urgently needed for the detection and control the dissemination of such organisms.
