Developing new therapeutic methods and exploring other possible future strategies in Triple Negative Breast Cancer (TNBC) is an area of interest. Angiogenesis is one such sought for area that can even serve as a targeted therapy in this subset of breast cancer. The objective of the study was to evaluate the prognostic factors in TNBC patients with emphasis on angiogenesis. Prospectively 120 patients with histologically confirmed breast cancer from June 2015 to July 2019 were included. The Colour Doppler evaluation of breast lump and axilla in terms of RI, PI and Vmax was done. Patients were categorized as having 'high' or 'low' RI, PI and Vmax on the basis of their mean value. Immunohistochemistry (IHC) was performed on viable tumour blocks obtained from mastectomy specimen using CD31 vascular endothelial staining. The idea was to obtain Microvascular Density (MVD) by counting all immunostained vessels at magnification of 400x. The association between TNBC and non TNBC with well-known Doppler parameters, tumour size, clinical lymph node status, number of positive lymph nodes, tumour grade, stage of disease and hormonal receptor status was investigated. Also the association between high and low MVD with these prognostic parameters were evaluated. A total of 120 patients were included in the study with the mean age of 42.43±7.73 years (range 30-65 years). The mean RI, PI and Vmax were 0.92±0.26, 2.19±1.84 and 16.52±10.70 respectively. The comparison between TNBC and non-TNBC with prognostic parameters showed significant association with age of patients, duration of disease, use of oral contraceptive pills (>1 year), tumour size, histological grade, RI and MVD (p=0.041, p=0.011, p=0.002, p=0.029, p=0.026, p=0.014 and p=0.007 respectively). The MVD value >13.17 (high) was found in 45 (37.5%) patients while 75 (62.5%) patients had low MVD value (<13.17). The high MVD (>16.52) was significantly associated with tumour size (p<0.001), axillary lymph node (p=0.022), clinical stage (p=0.015), histological grade (p<0.001), RI (p<0.001), ER status (p<0.001) and HER2 over expression (p=0005). TNBC is a subset of breast cancer showing aggressive biological behaviour as seen by presence of poor prognostic marker and increased vascularity.

BACKGROUND Breast cancer is the most common malignancy in women worldwide. The current diagnostic and management practices in breast cancer takes into consideration the neoplastic cells only and the tumour stroma is not addressed properly. Recent studies have shown that tumour stroma play an important role in tumourigenesis, its behaviour and response to therapy. In this study, we are trying to describe the histopathological changes in tumour stroma with respect to tumour stroma ratio, microvessel density and tumour associated macrophages. Aims and Objectives-To describe the histopathological changes in invasive carcinoma breast with respect to tumour stroma ratio (TSR), tumour associated macrophages (TAMs) and angiogenesis (Microvessel Density, MVD) Settings-Department of Pathology, Government Medical College, Kottayam. Design-Descriptive study. MATERIALS AND METHODS Study was conducted in first 134 biopsy specimens of invasive carcinoma breast received in the Department of Pathology, during the study period of 16 months (June 2017 to September 2018). Analysis-SPSS software. RESULTS In the present study, 66.4 % cases were of stroma low (stromal density < 50 %) type and 33.6 % cases were of stroma high type (Stromal density ≥ 50 %), comparable with the study conducted by A M Moorman et al. 59 % were found to have low microvessel density (MVD) and 41 % high MVD. Equal proportion of cases were found in TAM low and TAM high group. The association was found to be statistically significant between stromal density and microvessel density (p value 0.042), and also between MVD and TAM (p value of 0.022). CONCLUSION No statistically significant association was detected between age, tumour grade, stage of the tumour, lymph node metastasis, or molecular subtypes with stromal density, microvessel density and tumour associated macrophage density. Statistically significant association was found between microvessel density with stromal density and tumour associated macrophages.

The relationship between HER2 overexpression and angiogenesis in gastric cancer

In gastric cancer, HER2 protein overexpression is considered to be conducive to the higher proliferation activity of the tumor cells. Tumor formation is associated with angiogenesis in order to secure an abundant supply of oxygen and glucose to cancer cells. The aim of the study was to assess if HER2 overexpression is related to higher microvessel density (MVD) in the tumor stroma.The archival samples of primary tumor from 144 consecutive patients that underwent gastric resection for cancer between August 1, 2006 and December 31, 2013 in the Department of Oncological Surgery of Medical University of Gdańsk were analyzed. CD34 was used as a marker of MVD in the tumor stroma. Both CD34 and HER2 protein expressions were tested by immunohistochemistry.The assays were unsuccessful to estimate HER2 in 10 cases and CD34 in 14 cases due to technical reasons. The results were obtained for 128 patients. HER2 0 and HER2 1+ were considered negative, while HER2+ and HER2 3+ were recognized as positive. Mean MVD (mean number of vessels in the visual field) was 32.4 (median 29.5). Microvessel density was insignificantly higher in HER2 positive tumors. The slight difference was also seen between IHC 2+ and 3+ groups. The differences did not reach the level of statistical significance.Statistical analysis performed in our study did not reveal the significant relationship between HER2 overexpression on the tumor cells and MVD in the tumor stroma.
