Comparison of Light Transmission Aggregometry With Impedance Aggregometry in Patients on Potent P2Y12 Inhibitors

Abstract: Since data on the agreement between light transmission aggregometry (LTA) and multiple electrode aggregometry (MEA) in patients on the more potent P2Y12 inhibitors are missing so far, we investigated if the evaluation of the responsiveness to therapy by LTA can be replaced by MEA in 160 acute coronary syndrome (ACS) patients on dual antiplatelet therapy with aspirin and prasugrel or ticagrelor (n = 80 each). Cut-off values for high on-treatment residual platelet reactivity (HRPR) in response to adenosine dipho… Show more

“…The overall methodological quality of 14 cohort studies was generally high with the NOS scores ranged from 6 to 9 (Table 3). Six (27,31,40,(42)(43)(44) studies were rated six stars, three (24,28,41) were rated seven stars, four (23,38,45,46) were rated eight stars, and one (32) was rated nine stars.…”

“…Among included studies, 11 (25, 26, 29, 30, 33-37, 39, 47) studies were RCTs, 13 (23,24,27,31,32,38,(40)(41)(42)(43)(44)(45)(46) studies were prospective cohort studies, and one ( 28) study was retrospective cohort study. The publication dates of 25 studies ranged from 2012 to 2022.…”

“…Several investigations have linked HRPR to an increased risk of adverse ischemic outcomes. [60][61][62][63][64] While the laboratory response to P2Y12 inhibition can be improved by either increasing the dosage of clopidogrel or switching to the more potent P2Y12 inhibitors prasugrel or ticagrelor, [65][66][67] the intensification of DAPT based on platelet function testing did not result in improved clinical outcomes in most trials. [68][69][70] Accordingly, routine platelet function testing is not recommended by the current guidelines.…”
