Comparison of Intraoperative Indocyanine Green Angiography and Digital Subtraction Angiography for Clipping of Intracranial Aneurysms

Abstract: Background: Residual aneurysm after microsurgical clipping carries a risk of aneurysm growth and rupture. Digital subtraction angiography (DSA) remains the standard to determine the adequacy of clipping. Intraoperative indocyanine green (ICG) angiography is increasingly utilized to confirm optimal clip positioning across the neck and to evaluate the adjacent vasculature. Objective: We evaluated the correlation between ICG and DSA in clipped intracranial aneurysms. Methods: A retrospective study of patients who… Show more

“…For optical tumor imaging, Near infrared (NIR) dyes are favorable due to their deep penetration in tissues. Indocyanine green (ICG) is the only FDA approved NIR dye and has been used for full antibody labeling [ 33 , 34 ]. But its hydrophobic property limits its application in scFv labeling because scFv is not well tolerant to organic solvents.…”

“…[4,[7][8][9][10] The discordance between ICG-VA and intraoperative DSA reported by Caplan et al [11] was 10.8%, whereas Washington et al [12] reported this to be 14.3%. The incidence of discordance between ICG-VA and postoperative DSA reported by Doss et al [13] was 21.07%. In our study, the discordance between ICG-VA and postoperative DSA was 6.7% and is statistically significant (P <0.0001).…”

“…ICG angiography has complication rate of less than 0.1% 28 , 29 . The discordance between ICG and intraoperative digital subtraction angiography (DSA) in previous series were reported to be in the range of 10–20% 30 32 . A recent study has shown to be correlating with post-operative DSA findings in almost 97% of cases 23 .…”
