Comparison of Intralesional Corticosteroid and Propranolol Treatment of Periorbital Infantile Hemangiomas: An Outcome Study of 61 Cases

Abstract: The IC injections and oral propranolol medication equally improved amblyopia in children with IH of the periorbital and cheek region; however, propranolol was associated with fewer complications and additional or supportive treatment was not indicated. We prefer oral propranolol medication over IC injections in cases in which IH threatens to hamper visual acuity.

“…In 2008 Léaute‐Labrèze et al published a series of 11 cases of infantile haemangioma cases managed with propranolol which has revolutionized treatment. Two systematic reviews show both intra‐lesional steroids and propranolol are effective, though less side effects occur with propranolol . Propranolol is now the mainstay of treatment for periorbital infantile haemangioma and intra‐lesional steroids are used as an adjunct in resistant cases.…”

“…Glasman et al 19 reported wheezing in 23% of patients, and in 1 patient, wheezing was severe enough to require discontinuation of treatment. Hoornweg et al 20 reported cold extremities in 43% of patients. Painter and Hildebrand 21 reported mild wheezing in 1 patient.…”

“…In 2014, Hoornweg et al 20 reported on a retrospective case series comparing oral propranolol in 14 patients (maximum dose, 3 mg/kg daily) with intralesional triamcinolone (40 mg/ ml) in 29 patients. The size of the lesion was not measured.…”

“…The overall rate for side effects, based on 48 studies and 3,352 patients, was 7.1% (12, 34-67, 70-72, 74-77, 79, 80, 83-87). Six studies mentioned the need for additional treatment (local or systemic) for adverse events (38,40,45,52,54,69). The aggregated mean and median follow-up lengths were 17.09 and 12 months, respectively.…”
