Comparison of implementation strategies to influence adherence to the clinical pathway for screening, assessment and management of anxiety and depression in adult cancer patients (ADAPT CP): study protocol of a cluster randomised controlled trial

Abstract: BackgroundHealth service change is difficult to achieve. One strategy to facilitate such change is the clinical pathway, a guide for clinicians containing a defined set of evidence-based interventions for a specific condition. However, optimal strategies for implementing clinical pathways are not well understood. Building on a strong evidence-base, the Psycho-Oncology Co-operative Research Group (PoCoG) in Australia developed an evidence and consensus-based clinical pathway for screening, assessing and managin… Show more

“…This real-world feasibility study was helpful in fine tuning the ADAPT Portal and implementation strategies for the subsequent cluster randomised trial, which commenced in 2017, with final data collection envisaged for 2020 [15]. Conversations about terminology, hospital processes and flow of systems allowed the facilitators and staff to develop a shared language and pre-empted some barriers during the study.…”

“…We therefore selected this approach to meet the aims of the current study and used the consolidated criteria for reporting qualitative research (COREQ), a 32-item checklist for interviews and focus groups (see Additional file 1), to guide structure and reporting [17]. This feasibility study was designed to inform the multi-site cluster randomised trial, in which sites will be randomised to different levels of implementation support-full details of the main trial are available in the published protocol [15].…”

“…Full descriptions of the ADAPT CP (17, 18) and the ADAPT Cluster RCT protocol (19) are reported elsewhere but brief details are provided here to give context. The ADAPT Cluster RCT is registered with the Australian New Zealand Clinical Trials Registry ACTRN12617000411347.…”

“…There is a crucial need within the field of implementation science to better understand the specific mechanisms that contribute to successful implementation of healthcare innovations. Gaining an understanding of which implementation strategies do and do not work, and at what "dose," will ideally strengthen study designs, increase uptake of innovations, and ultimately improve health outcomes [1,2]. One key strategy needing examination is facilitation, the process of aiding uptake of innovations by helping individuals and teams to understand change processes and achieve implementation goals [3].…”
