IntroductionInfectious agents, mainly viruses, are among the few known causes of cancer and contribute to a variety of malignancies worldwide. The agents considered here, termed Epstein-Barr virus (EBV) and Kaposi sarcoma herpesvirus (KSHV-human herpesvirus 8 [HHV8]), are members of the gamma-herpesvirus subfamily. 1 Since its discovery as the first human tumor virus, EBV has been implicated in the development of a wide range of B-cell lymphoproliferative disorders, including Burkitt lymphoma (BL), nasopharyngeal carcinoma, and Hodgkin and non-Hodgkin lymphomas (NHLs). KSHV, one of the most recently discovered human tumor viruses and the cause of Kaposi sarcoma (KS), 2 also plays a role in the pathogenesis of primary effusion lymphoma [PEL], and multicentric Castleman disease [MCD]). [3][4][5][6] Intriguingly, EBV and KSHV have been shown to associate with distinct lymphoproliferative diseases occurring most often in persons with HIV infection/ AIDS 7,8 or in association with other immunodeficiency conditions, such as iatrogenic immunodeficiency following solid organ transplantation. 9,10 HIV-associated lymphoproliferative disorders are a heterogeneous group of diseases that arise in the presence of HIV-associated immunosuppression, a state that permits the unchecked proliferation of EBV-and KSHV-infected lymphocytes. Traditionally, these aggressive disorders mainly include both central nervous system and systemic lymphomas, 11 whereas lymphomas specifically occurring in the setting of HIV infection include PEL and its solid variant, plasmablastic lymphoma (PBL) of the oral cavity type and large B-cell lymphoma arising in KSHV-associated MCD. [12][13][14][15][16][17][18] Thus, HIV-related lymphomas are closely linked to EBV infection of the tumor clone or are associated with KSHV. PEL and its variants often involve EBV in addition to KSHV. 19 We review here the current knowledge on these gammaherpesvirus-associated lymphomas in the setting of HIV infection. The focus will be on pathology, diagnosis and classification, pathogenesis, and treatment of these lymphomas specifically occurring in HIV-induced immunodeficiency. JC EH lin xp ematopathol Case Study INTRODUCTIONChronic myelogenous leukemia (CML) is a chronic myeloproliferative neoplasm that originates from multipotent hematopoietic stem cells and is associated with the BCR-ABL fusion gene located on the Philadelphia (Ph) chromosome.1 Based on historical data prior to any effective therapy, median survival times for CML ranged between 2 and 3 years. 2 However, in the current era of tyrosine kinase inhibitor (TKI) therapy using imatinib, dasatinib, nilotinib, and bosutinib, the prognosis has been markedly improved. 3 Currently, an important issue is the management of adverse events. Pleural effusion is one of the major problems. The TKIs have different patterns of side effects, and pleural effusion is more frequently observed with dasatinib compared with imatinib, nilotinib, and bosutinib. [4][5][6][7] Primary effusion lymphoma (PEL) is a rare type of nonHodgkin lymphoma (NHL) confined to the body cavities, usually without a tumor mass.1 It is associated with infection of human herpesvirus 8 (HHV-8), and Epstein-Barr virus (EBV) coinfection is common, especially with a background of human immunodeficiency virus type-1 (HIV) infection. 8 However, some cases of HHV-8-negative primary lymphomatous effusion have been reported and termed HHV-8-unrelated PEL-like lymphoma. 9,10 In this report, we present a rare case of PEL-like lymphoma that was found during pleural effusion treatment with TKIs for CML. CASE REPORTA 69-year-old Japanese male was hospitalized for cerebral infarction. Peripheral blood evaluation revealed a hemoglobin level of 12.2 g/dl, a platelet count of 98.6 x 10⁴ / μl, a white blood cell count of 26,800 /μl with 86% neutrophils, 6% lymphocytes, 1% monocytes, 1% eosinophils, 2% basophils, 2% myelocytes, and 2% metamyelocytes. Hepatitis B, Hepatitis C, HIV, and HTLV-1 were negative. Hypercellular bone marrow with an increase of megakaryocytes was observed on the bone marrow test, and the myeloblast count was less than 5% (Figure1). Chromosomal analysis was abnormal with 46, XY, t(9;22)(q34;q11.2) [20/20]. The patient was diagnosed with CML in the chronic phase. Dasatinib was started at 100 mg once daily. After a year of dasatinib treatment, the patient achieved major molecular response (MMR). However, he developed dyspnea and bilateral pleural effusion was observed on chest X-ray; therefore, diuretics were started. We report two cases of human herpesvirus-8 (HHV-8)-negative large B-cell lymphoma involving pericardial and/or pleural effusion that regressed after drainage alone. Case 1 is a 70-year-old man showing massive pericardial effusion. Cytology of the drained effusion showed monotonous infiltration of CD3-, CD20+, CD79a+, and CD138- large B-cells. Monoclonality was shown by Southern blot analysis. Case 2 is a 70-year-old man with massive pericardial and bilateral pleural effusion. Cytology of pericardial effusion showed infiltration of CD20+, CD45RO-, CD138-, immunoglobulin lambda chain+, and kappa chain- large B cells. In both cases, effusion resolved after drainage and no relapse has been observed. HHV-8 was not demonstrated in either case. Clinical presentation of our two cases resembled primary effusion lymphoma (PEL), but cytomorphology, immunophenotype, and prognosis were clearly distinct from those of PEL. HHV-8-negative effusion lymphomas might include prognostically favorable self-limited tumors that could regress without any cytotoxic therapy.
