Comparison of Goldmann applanation tonometry with the Tonopen for measuring intraocular pressure in a population-based glaucoma survey in rural West Bengal

Abstract: The overall agreement between the measurements of the two instruments was good but a small percentage of large difference (> or = +/-5 mm Hg in 7.4%) may be of concern in a population-based survey. The time to measure IOP and the subjects' preference were strongly in favour of the Tonopen.

“…However, we found substantial agreement between the 2 modes of IOP measurement, as has been reported elsewhere. 38 This argues against systematic error due to instrumentation. We estimated the heritability of CDR to be 0.56, which compares well with the previous reported values of 0.48 to 0.80. 24,26 It is not surprising that a simple cross-sectional measurement of CDR should show higher heritability than IOP, which exhibits substantial diurnal variation and is more sensitive to the effects of environmental influences such as drugs.…”

“…The hospital setting had more patients with high IOP ranges and measurement of IOP with a Tonopen is known to be less accurate at extreme values. 30,31 There was moderate level of agreement (k ¼ 0.52, s.e. ¼ 0.0359) on VCDR assessment in the community setting.…”

“…In eight studies 75,107,140,144,153,157,166,169 (53%), the tonometers used were calibrated. Almost all studies 107,138,140,144,153,157,166,188,191,197,206,207,216 (13, 87%) included all participants approached in the analysis or stated a reason why not. The Salvetat 144 study met all of the specified criteria.…”
