Background and objectives: Cytomegalovirus (CMV) is one of the most significant agent of infection in immunocompromised patients and transplant recipients. CMV is transmissible through transfusions of blood components. The present study amied to determine the seroprevalence of Cytomegalovirus (CMV) among voluntary blood donors in Erbil province, North of Iraq. Methods: A prospective study was done on serum samples from 472 HIV, HCV, and HbsAg-seronegative healthy male voluntary blood donors were screened for CMV IgM /IgG using Enzyme-Linked Immunosorbent assay (ELISA). Results: A total of 472 healthy voluntary blood donors were recruited; 148(31.36%) of the donors were anti-CMV IgG positive. There were no statistically significant differences between CMV seropositivity and age. Further, seven (1.48%) subjects were both anti-CMV IgM and IgG positive. Conclusions: The moderate seroprevalence of CMV IgG antibodies in the our donor population, underscores the importance of using strategies such as leukoreduction and transfusion with CMV-seronegative blood is recommended for preventing and reducing transmission of CMV with the infected blood or blood products in immunocompromised patients.

DNA fragments from eight different reading frames of human cytomegalovirus (HCMV) were generated by PCR and subsequently cloned and expressed in Escherichia coli in fusion with glutathione S-transferase. The recombinant viral antigens were evaluated in immunoblot analyses. The most reactive antigens were purified and further evaluated in ELISAs. For this, sera from healthy blood donors and immunocompetent individuals with acute HCMV infection, and follow-up sera from transplant recipients with acute primary HCMV infection were used. The results of our experiments indicate that only three particular recombinant polypeptides from two viral proteins are necessary for serodiagnosis. While a fragment covering amino acids (aa) 495 to 691 of ppl50 (150/1) was the most suitable antigen for the identification of infected individuals in general, immunoglobulin M antibodies against the C-terminal parts of ppl50 (aa 862 to 1048; 150/7) and p52 (aa 297 to 433; 52/3) proved to be excellent serological markers to monitor acute HCMV infection. The selected recombinant antigens enable the improvement of serodiagnosis of HCMV-related diseases, especially during the early stages of infection.

Background: Chronic haemodialysis patients are at high risk for infection because the process of haemodialysis (HM) requires vascular access for prolonged periods. The human Cytomegalo Virus (CMV) is a DNA virus belonging to the Herpesviridae family, Betaherpsvirnae subfamily and Cytomegalo virus gender, known as Human Herpes Virus Type 5, one of the main causes of morbidity. Acute CMV infection is characterized by system-wide viremia after which latency and lifelong persistence is established in selected cells such as CD34+ monocytes and hematopoietic progenitor cells in humans. Objective: The work was designed for early detection of CMV infection and metabolic syndrome (MetS) in patients on HD.Patients and Methods: In this cross-sectional study, 80 patients under regular HD due to end-stage renal disease (ESRD) had participated in the study from HD Unite of Nasr City Health Insurance Hospital in Cairo in the period from June 2019 to December 2019. Results: In the present study, patients were divided into three groups, first group 78 patients were positive for CMV IgG (97.5%), second group 11 patients were positive for CMV IgM (13.8%) and third group 10 patients were positive for both IgG and IgM (12.3%). In our study the females have more CMV antibodies. In the present study, patients with metabolic syndrome were 68 (85%). Patients who had MetS and CMV IgG were 68 (100%). The relationship between MetS and CMV IgG is statistically proved. Conclusion: the importance of early investigation for metabolic syndrome components especially in patients who are immunocompromised like HD patients because of the high prevalence of both chronic CMV infection and reactivation.
