Purpose: To investigate the clinical efficacy of Gushukang Jiaonang in the prevention and treatment of osteoporosis, and its effect on patients' bone metabolism based on its use in traditional Chinese medicine.Methods: Retrospective analysis was conducted on 108 patients with osteoporosis discharged from the North Hospital Affiliated Hospital of Gansu University of Traditional Chinese Medicine, from January 2021 to January 2023. Patients were divided into study group (n = 54) and control group (n = 54). Control group received oral calcitriol and calcium tablets, while study group orally received calcitriol and calcium tablets and Gushukang Jiaonang capsules in addition. Clinical symptom improvement scores, bone mineral density, serum phosphorus and calcium, and blood alkaline phosphatase levels were compared between the groups.Results: After treatment, total amino terminal prolonging peptide of collagen I in the blood of study group was significantly higher than before treatment (p < 0.05). Osteocalcin was significantly increased before and after treatment in the two groups (p < 0.05). Vitamin D levels were significantly increased before and after treatment, and the difference was statistically significant (p < 0.05).Conclusion: Gushukang Jiaonang provides patients with efficient, comprehensive and precise prevention and treatment of osteoporosis. It also promotes positive disease regression, which in turn contributes to increase in bone mineral density in individuals. Long-term investigations with a larger population size, more indices as well as multicenter control should be carried out to provide sufficient theoretical basis for use in clinical practice.

Receptor activator of nuclear factor κB (RANK) and its ligand (RANKL) play key roles in bone metabolism and the immune system. The RANK/RANKL complex has also been shown to be critical in the formation of mammary epithelia cells. The female hormones estradiol and progesterone closely control the action of RANKL with RANK. Blood concentration of these sex hormones in the postmenopausal period leads to an increase in RANK/RANKL signaling and are a major cause of women’s osteoporosis, characterized by altered bone mineralization. Knowledge of the biochemical relationships between hormones and RANK/RANKL signaling provides the opportunity to design novel therapeutic agents to inhibit bone loss, based on the anti-RANKL treatment and inhibition of its interaction with the RANK receptor. The new generation of both anti- and mesoprogestins that inhibit the NF-κB-cyclin D1 axis and blocks the binding of RANKL to RANK can be considered as a potential source of new RANK receptor ligands with anti-RANKL function, which may provide a new perspective into osteoporosis treatment itself as well as limit the osteoporosis rise during breast cancer metastasis to the bone.

Background: The ultrasonic backscatter (UB) has the advantage of non-invasively obtaining bone density and structure, and it is expected to be an assessment tool for osteoporosis early diagnosis. The previous UB measurements were all based on exciting a short single-pulse and analyzed the ultrasonic signals backscattered in bone. Purpose: This study aims to investigate an amplitude modulation (AM) ultrasonic excitation with UB measurements for estimating bone characteristics. Methods: The AM multiple lengths excitation and backscatter measurement (AM-UB) functions were integrated into a portable ultrasonic instrument for bone characterization. The apparent integrated backscatter coefficient in the AM excitation (AIBAM) was calculated on the AM-UB instrumentation. The correlation coefficients of the AIBAM estimating volume fraction (BV/TV), structure model index (SMI), and bone mineral density (BMD) were analyzed. Results: Significant correlations (|R| = 0.82 – 0.93, p < 0.05) were found between the AIBAM, BV/TV, SMI, and BMD. When increasing the AM excitation length, the AIBAM values show more stabilized both in 1.0 MHz and 3.5 MHz measurements. The recommendations in AM-UB measurement were that the avoided length (T1) should be less than AM excitation length, and the analysis length (T2) should be appropriately long but not more than AM excitation length. Conclusions: This study conducts an AM-UB measurement for cancellous bone characterization. Increasing AM excitation length can significantly improve AIBAM values stability with varied analyzed signals. The study suggests the portable AM-UB instrument with the integration of real-time analytics software that might provide a potential tool for osteoporosis early screening.
