Comparison of dobutamine stress echocardiography, dobutamine SPECT, and adenosine SPECT myocardial perfusion imaging in patients with end-stage renal disease

Abstract: There is moderate concordance between DSE and adenosine SPECT MPI in ESRD patients referred for stress testing. Interobserver agreement was higher for SPECT MPI compared to DSE. Based on these observations, the optimal approach for diagnosing severe coronary artery disease and assessing risk in patients with ESRD has yet to be determined, but appears to warrant further investigation.

“…Routine angiography, even by computed tomography scanning, has risks and, even in the best centers, the intervention rate is low. 5,6 One underlying reason is that coronary artery disease is only a part of the cardiovascular disease profile of patients with ESRD, in which uremic cardiomyopathy (the principal form of which is ventricular hypertrophy, with myocardial fibrosis) is an important determinant of sudden cardiac death and heart failure. 1,2,7 On the other hand, it is clear from screening programs that exercise capacity in potential transplant recipients is a strong predictor of post-transplant outcomes.…”

“…The presence of coronary flow on post‐stress images and its absence on rest images represent an area of ischemia. The test sensitivity is often greater for MPI, while specificity is slightly higher for DSE .…”

“…When evaluating two identical physiological tests (nuclear SPECT) for the detection of ischemia performed using the same vasodilator (adenosine) in two different settings, the agreement is moderate at 62 % with a κ value of 0.46[ 16 ]. Simultaneous evaluation for two different imaging modalities (echocardiography and nuclear SPECT) during the same dobutamine stress evaluation have demonstrated an agreement of 69 % with a κ value of 0.25[ 17 ]. Thus, the agreement between CTA and CMR in the current study is excellent compared with the concordance of conventional tests.…”
