Comparison of Costs of Endovascular Repair versus Open Surgical Repair for Abdominal Aortic Aneurysm in Korea

Abstract: This study was designed to compare the hospital-related costs of elective abdominal aortic aneurysm (AAA) treatment and cost structure between endovascular aneurysm repair (EVAR) and open surgical repair (OSR) in Korean health care system. One hundred five primary elective AAA repairs (79 OSRs and 26 EVARs) performed in the Seoul National University Hospital from 2005 to 2009 were included. Patient characteristics were similar between two groups except for older age (P = 0.004) and more frequent history of mal… Show more

“…Our claim of a real-world, nationwide representation is substantiated by the results of our calculated mean total cost, which is consistent with the results of the National In-patient Sample, 32,37 24,38 Our cost data are synchronous with index hospital admission data of American studies. 31,32,38,39 Interestingly, our results also contrast Brazilian, 26 Korean, 40 and Japanese 41 studies that suggest EVAR is costlier than OAR, which is probably owing to countrywise differences 42 in procedural application and cost of hospital stay. 20 Similar to any retrospective administrative database study, our study had several limitations, including erroneous coding or improper data entry, leading to systemic bias.…”

“…Three studies that included Korean and/or Chinese patients were excluded from relative survival analysis because reliable life expectancy tables were not available. Two studies that did not report the number of patients at risk in survival analysis were also excluded.…”

“…On the contrary, the EVAR technique became the preferred method for uAAAs since 2008, consistent with the literature [16] . The advantage of this surgical procedure is that it requires shorter hospitalizations (on average 5 days less), although it is more costly [15] , [36] . However, the adjusted mortality did not decrease since the adoption of EVAR technique, in line with Gupta et al [37] that used prospectively collected data from the National Surgical Quality Improvement Programme (for patients <60 years).…”
