Comparison of bronchial and nasal allergen provocation in children and adolescents with bronchial asthma and house dust mite sensitization

Abstract: Background: Bronchial allergen provocation (BAP) is an established tool for the diagnosis of allergy in patients with asthma, but its use is limited by the potential risk of severe asthmatic reactions. Nasal provocation testing (NPT) may be an alternative safe method and does not require sophisticated equipment. Objective: The aim of this prospective study was to evaluate the concordance of both methods in patients with asthma and house dust mite (HDM) sensitization. Methods:A total of 112 patients with HDM sen… Show more

“…1 Its pathophysiology, which involves various cells and inflammatory factors, 2 is mainly characterized by airway hyperresponsiveness, inflammation, and smooth muscle spasm, as well as goblet cell proliferation and increased mucus secretion, 3,4 It was estimated that approximately 300 million people worldwide experience bronchial asthma 5 ; moreover, along with the acceleration of urbanization in our country, the incidence of bronchial asthma is increasing annually. 6 Relevant research has indicated that bronchial asthma has the highest incidence rate among all pediatric chronic diseases 7 ; repeated acute asthmatic crises can severely affect children’s quality of life, ultimately threatening their health and substantially burdening their families and the society. At present, clinical treatment is mainly based on pharmacological control, which can hardly cure asthma.…”

“…Of note, 83-89% of atopic asthmatics with rhinitis who tested positive to the nasal challenge with HDM have positive BAC results. 44,93 The NAC is a safe and reproducible technique 94,95 with a validated methodology 56 and defined cut-off points for positivity. 96 Moreover, challenge protocols are considerably shorter than those of BAC.…”

“…In this regard, NAC could be a useful tool to phenotype the inflammatory disease affecting the airways regardless of its organ‐specific manifestations (conjunctivitis, rhinitis and asthma). Of note, 83–89% of atopic asthmatics with rhinitis who tested positive to the nasal challenge with HDM have positive BAC results 44,93 . The NAC is a safe and reproducible technique 94,95 with a validated methodology 56 and defined cut‐off points for positivity 96 .…”

“…The NPT was performed according to our laboratory's standard as recently described in more detail and interpreted in concordance with international standards. 4,5,20,21 Before starting the NPT, we made sure that no patient had used any treatment for rhinitis before. After nasal secretion collection, a NPT with HDM allergen was performed.…”
