Purpose: To evaluate if latex coated polypropylene mesh leads to less adhesions formation. Methods: 90 rats were distributed in three groups. Group I (control) was submitted to median laparotomy and posterior synthesis; Group II (polypropylene) was submitted to a segment resection of the abdominal wall, and the defect was corrected with a polypropylene mesh; Group III using the new material (latex coated polypropylene). After 45 days the euthanasia procedure was done and the adhesions were evaluated in 2 ways: 1) classification in a grading system; 2) evaluation of the adhesion area, which was calculated by resection of the adhesions after dying with Indian blue ink. Results: The classification in a grading system showed that Group III animals had less adhesions formation (P<0.05), but the evaluation of the adhesion area did not show any difference (P>0.05). The severe complications (fistula, evisceration and obstruction), which were lethal, were not reduced by the latex coated material. Conclusion: Latex coated polypropylene causes fewer adhesion formations than polypropylene alone, although it does not diminish the severe early complications. Key words: Polypropylenes. Latex. Surgical Mesh. RESUMO Objetivo:Avaliar se o revestimento da tela de polipropileno com látex leva a menor formação de aderências. Métodos: Foram utilizados 90 ratos, distribuídos em três grupos de 30: grupo I (controle) animais submetidos a laparotomia mediana e posterior síntese; grupo II (polipropileno) ressecção de segmento da parede abdominal e síntese com tela de polipropileno; grupo III, (polipropileno revestido por látex). Eutanásia foi realizada após 45 dias e as aderências avaliadas sob duas formas: 1) classificação em graus; 2) avaliação da área aderida, calculada corando-se o segmento da parede contendo as aderências que posteriormente foram ressecadas produzindo uma região clara em contraste com o restante da peça, corada. Resultados: Na avaliação por meio de graus, o grupo III apresentou menor formação de aderências (p<0,05), mas quanto a avaliação da área aderida não houve diferença (p>0,05). As complicações graves não foram reduzidas pelo revestimento por látex (p>0,05). Conclusão: O revestimento da tela de polipropileno por látex leva a menor formação de aderências, porém não diminui as complicações graves precoces.

Abstract:Objective: In this study, we aimed at investigating the effect of placing hyaluronate-carboxymethylcellulose membrane (HCMC) on the formation of adhesion postoperatively in a damaged area in the peritoneum of the anterior stomach wall. Methods: The study was conducted on 30 rabbits. A transverse peritoneal damage was infl icted on the stomach anterior walls of all rabbits. In the fi rst treatment group, HCMC was placed on the sutured anterior wall of stomach of 15 rabbits. In the second control group, on the other hand, no treatment was conducted on 15 rabbits. On the 30th day after the operation, relaparatomy was performed on the rabbits and adhesions were evaluated by an independent surgeon according to seriousness and prevalence scores. Results: There were postoperative adhesions (POA) in 12 (80 %) rabbits in the control group. On the other hand, there were POA in 5 rabbits (33.3 %) in the treatment group. In the treatment group, adhesion was totally prevalent in 2 rabbits (13.3 %), whereas this ratio was 7 (46.6 %) in the control group (p < 0.01). Conclusions:The study suggested that the use of hyaluronate-carboxymethyl-cellulose could be benefi cial on damaged peritoneum surfaces following abdominal surgery in order to reduce POA development to a minimum (Tab. 3, Fig. 3, Ref. 22). Background: Physical barriers and instilled solutions have been studied to prevent intra-abdominal adhesions. However, undesirable side effects of these substances on the healing of intestinal anastomoses may limit their use. This study was designed to compare the effects of antiadhesives on the healing of colonic anastomosis in rats. Methods: Sixty female Sprague-Dawley rats were divided into 3 groups of 20. The animals received isotonic saline and 7.5% icodextrin, intraperitoneally after standard left colonic anastomosis. In group 3, Seprafilm® was wrapped around the anastomosis and also laid over the abdominal viscera. Half of the animals from each group were killed on postoperative day (POD) 4 and the remaining half on POD 21. Adhesion scoring, bursting pressure and tissue hydroxyproline measurements and histopathological assessment were performed. Results: Mean hydroxyproline levels were significantly higher in groups receiving icodextrin and Seprafilm compared with the control group, whereas mean bursting pressures were significantly higher in the group that received icodextrin (p < 0.05). Intraperitoneal administration of icodextrin resulted in significant reduction of adhesion formation on POD 21 (p < 0.05). Conclusions: Seprafilm does not prevent formation of adhesions as much as icodextrin does, but its effect on the healing of colonic anastomoses is similar.
