Comparison of a Novel Liquid (Adcon-P®) and a Sodium Hyaluronate and Carboxymethylcellulose Membrane (Seprafilm™) in Postsurgical Adhesion Formation in a Murine Model

Abstract: This study suggests that both Adcon-P and Seprafilm trade mark decrease the incidence of postoperative adhesions and the difficulty of adhesiolysis in the murine cecal abrasion model. However, Adcon-P appeared to be superior to Seprafilm. This agent is an attractive device that requires additional studies.

“…Therefore, if the literature is controversial in relation to the protector or non-protector role of the adhesions over the anastomoses, it is consensual that its excessive formation and principally the ones that involve the abdominal wall can be damaging, and these are the ones we intend to reduce with prostheses coating. Many experimental studies evaluate the adhesions using a grading system 12,18,19 that, apart from being subjective, may not be advantageous in clinical practice. Therefore, this quantification, concomitant with two other forms of studying adhesions in an objective way (calculation of the adhesion area and observation of complications and deaths), is a more careful proceeding.…”

“…The animals that died because of anesthesia problems, bleeding and accidents during the surgery were excluded. The animals were weighted and anesthetized by intramuscular injection of 30mg/Kg of ketoamine hydrochloride and 0.5mg/Kg of xylasine hydrochloride 18 . Afterwards they were submitted to abdominal trichotomy and anti-sepsis with 2% iodized polyvinylpirrolidone, with the placement of sterile zones.…”

“…In a period of over 10 years, Seprafi lm® use saves 383 dollars for the patient and 1122 dollars for each patient socioeconomically (19). The fact that Seprafi lm® is thin and easily wrinkles, causes diffi culty in application, especially in abdomens that are hard to recover, and in small incisions (20). The ideal adhesion barrier should meet the following criteria: (1) it should separate tissues effectively; (21) it should have a half-life that is long enough to stay in peritoneum for the critical 7 day period, which is the time for peritoneum to heal; (7) it should be absorbed and metabolized without triggering an obvious infl ammatory tissue response; (8) it should stay effective and active when there is blood in the environment; (3) it should not interrupt healing of the scar; (22) and it should not contribute to bacterial increase (3).…”

“…There are contradictory data about the effects of Seprafilm on the healing of intestinal anastomoses in experimental studies. Some authors reported that Seprafilm had adverse effects on anastomotic healing, with an increased rate of abscess formation and anastomotic leakage [6,20,32] , whereas others reported that Seprafilm did not interfere with healing of intestinal anastomoses [17,29,33,34] . In our study, hydroxyproline values were significantly higher in the icodextrin and Seprafilm groups (p !…”
