Comparison of a Custom-made and a Thermoplastic Oral Appliance for the Treatment of Mild Sleep Apnea

Abstract: In this study, a custom-made device turned out to be more effective than a thermoplastic device in the treatment of SDB. Our results suggest that the thermoplastic device cannot be recommended as a therapeutic option nor can it be used as a screening tool to find good candidates for mandibular advancement therapy.

“…We advanced the mandible manually, aiming to achieve an optimal effect under pharyngoscopic guidance. This technique was chosen after failure of a thermoplastic oral appliance to produce adequate tongue advancement in preliminary trials, in accordance with a recent publication (28). Although the manual technique could be unstable, we found no evidence for consistent instability, and considered the necessity to obtain optimal MA to evaluate the effect of GG-ES at this condition more important.…”

“…MA has been shown to decrease Pcrit during sleep (9) and muscle paralysis (11) and to enlarge both the oro-and velopharynx (11). However, MA produces only a partial clinical effect in OSA, even when custom-made devices are used (28).…”

“…26 We are aware of one previous study which addressed the feasibility of using MAS as an adjunct treatment in patients previously established on CPAP. 12 The study used a non-adjustable MAS at 75% of mandibular advancement.…”

“…3 In addition, failure rates with thermoplastic appliances appear to be very high when compared to custom-made appliances. 4 Direct comparisons of OAs use, CPAP therapy, or behavioral modifications (such as improved sleep hygiene and weight loss) suggest that only CPAP and OAs are effective in significantly reducing the ApneaHypopnea Index (AHI). In mild subjects (AHI 5-15), both CPAP and OAs exhibit significant improvements in AHI, sleepiness, and functional status.…”
