Objective:Evaluating the effects of heart cavity volume, presence and absence of perfusion defect, gender and type of study (stress and rest) on the difference of systolic parameters of myocardial perfusion scan in 16 and 8 framing gated SPECT imaging.Methods:Cardiac gated SPECT in both 16 and 8 framing simultaneously and both stress and rest phases at one-day protocol was performed for 50 patients. Data have been reconstructed by filter back projection (FBP) method and left ventricular (LV) systolic parameters were calculated by using QGS software. The effect of some factors such as LV cavity volume, presence and absence of perfusion defect, gender and type of study on data difference between 8 and 16 frames were evaluated.Results:The differences in ejection fraction (EF), end-diastolic volume (EDV) and end-systolic volume (ESV) in both stress and rest were statistically significant. Difference in both framing was more in stress for EF and ESV, and was more in rest for EDV. Study type had a significant effect on differences in systolic parameters while gender had a significant effect on differences in EF and ESV in rest between both framings.Conclusion:In conclusion, results of this study revealed that difference of both 16 and 8 frames data in systolic phase were statistically significant and it seems that because of better efficiency of 16 frames, it cannot be replaced by 8 frames. Further well-designed studies are required to verify these findings.

Coronary Artery Calcium Is Associated with Left Ventricular Diastolic Function Independent of Myocardial Ischemia

It has been shown in several studies that coronary artery calcium (CAC) burden or CAC progression is associated with heart failure. We tested the hypothesis that the extent of CAC is associated with left ventricular (LV) diastolic parameters derived from gated myocardial perfusion single-photon emission computed tomography (SPECT) in patients with no evidence of myocardial ischemia. 157 patients undergoing coronary computed tomography (CT), gated SPECT, and transthoracic echocardiography (TTE) were enrolled in this study. The CAC score was calculated according to the Agatston method. The peak filling rate (PFR) and the one-third mean filling rate (1/3MFR) were obtained as LV diastolic parameters. There were 139 patients with CAC and 18 patients without. The CAC score ranged from 0 to 4,976. There were no significant differences in the LV end-diastolic volume (LVEDV) (61 ± 21 mL versus 62 ± 22 mL, P = 0.79) and LV ejection fraction (LVEF) (66 ± 9% versus 68 ± 9%, P = 0.43). Patients with CAC had lower PFR than those without (2.2 ± 0.5 EDV/s versus 2.6 ± 0.7 EDV/s, P = 0.03). Multivariate linear regression analysis showed that ln (CAC score + 1) was significantly associated with PFR (β = −0.20, P = 0.01) and 1/3MFR (β = −0.18, P = 0.049). Our data suggest that the extent of CAC is inversely associated with LV diastolic parameters derived from gated SPECT independent of myocardial ischemia.

Установка имплантируемого кардиовертера-дефибриллятора (ИКД) пациентам с факторами риска является одним из основных способов профилактики внезапной сердечной смерти (ВСС) в результате возникновения ЖТА. В то же время показано, что срабатывания данного устройства на протяжении 5 лет наблюдения регистрируются лишь у 15-25% больных. В связи с тем, что установка ИКД ассоциирована с госпитализацией и инфекционно-воспали-тельными осложнениями, существует необходимость совершенствования стратификации риска и поиска новых прогностических маркеров ВСС и ЖТА, для того чтобы уменьшить количество нерационально имплантированных кардиовертеров-дефибрилляторов и снизить необоснованные затраты на госпитализацию и оперативное вмеша-тельство. В связи с этим актуален поиск новых предикторов развития ЖТА. Перспективным в этом направлении является выявление перфузионных нарушений миокарда по данным сцинтиграфии с таллием-199, нарушения симпатической иннервации миокарда, оцененные сцинтиграфией с метайодбензилгуанидином (МИБГ), меченным йодом-131, а также определение уровня потенциальных биохимических маркеров фиброза миокарда, таких как галектин-3 и ST2. At the same time, data of 5 years of follow-up show that such kind of device is effective only in 15-25% of patients. For the reason that ICD implantation is associated with hospitalization and infection complications, there is a need to improve risk stratification and search for new prognostic markers of SCD and VA in order to reduce the number of inefficiently implanted ICDs and to reduce the costs of surgical intervention.
