Objective Intravesical prostatic protrusion (IPP) is a noninvasive parameter that can predict disease progression and development of complications in benign prostatic hyperplasia (BPH). This study was to determine the relationship between IPP and the presence of complications in BPH patients. Methods This was a cross‐sectional study of BPH patients at Enugu State University of Science and Technology Teaching Hospital, Enugu. Patients were assessed for acute urinary retention (AUR), chronic urinary retention (CUR), epididymoorchitis, hematuria, hernia, urinary tract infection (UTI), serum creatinine, and prostate‐specific antigen (PSA). They also had abdominal ultrasonography assessments for IPP, total prostate volume, bladder wall thickness (BWT), postvoid residual (PVR), hydronephrosis, bladder diverticulum, and urolithiasis using Sonoscape S11 with an abdominal‐probe frequency of 3.5 MHz. IPP was measured in millimeter and divided into <10 mm and ≥10 mm. Data were analyzed using SPSS version 21 and were subjected to 1‐way analysis of variance, chi‐square test, and Pearson correlation. The odds ratios of development of complications at an IPP cutoff of 10 mm were calculated. P < .05 was considered significant. Results A total of 118 patients with a mean age of 64.18 ± 10.96 years and a mean IPP of 14.29 ± 10.20 mm were included. Forty‐eight patients had IPP < 10 mm and seventy patients ≥10 mm. Patients with IPP ≥10 mm had significantly higher mean BWT, International Prostate Symptom Score (IPSS), PSA, and PVR (P ≤ .05) and significantly more AUR, CUR, hematuria, hydronephrosis, and UTI (P ≤ .01). Conclusion Patients with IPP ≥ 10 mm have a significantly higher incidence of some complications.

Introduction: Benign prostatic hyperplasia is a health problem whose incidence ranks second highest in the field of urology. One of the cause is the more severe the complaint that arises is the heavier prostat volume which can cause the iritative and obstructive symptoms or better known as lower urinary tract symptoms(LUTS) which will be assessed based of international prostate symptoms score (IPSS). This study aims to determine prostate volume correlation with lower urinary tract symptoms severity in RSUD dr. Zainoel Abidin Banda Aceh. Knowledge of the volume of the prostate is important in management of patients with benign prostatic hyperplasia.Method: This study uses the Cross Sectional design by selecting respondents in accidental sampling with a total of 31 patient. The data obtained were then analyzed by the Spearman test.Result: The univariat data shows that 18 patient (58.1%)Â are in Grade III (31-50 cc) and the most coming patient have LUTS scoring symptoms level on moderate (8-19) are 12 patient (38.7%) and severe symptoms (20-35) are 17 patient (54.8%). The results of prostate volume with lower urinary tract symptoms data analysis show that the p-value = 0.323 (p>0.05).Conclusion: The results of this study indicate that the prostate volume doesnâ€™t have correlation with lower urinary tract symptoms severity in patient with benign prostatic hyperplasia in RSUD dr. Zainoel Abidin Banda Aceh.Â Latar Belakang: Benign prostatic hyperplasia (BPH) merupakan masalah kesehatan yang insidensinya menempati urutan kedua tertinggi di bidang urologi. Salah satu penyebab semakin beratnya keluhan yang muncul adalah semakin besarnya volume prostat dan dapat menyebabkan gejala klinis berupa gejala iritatif dan obstruktif atau lebih dikenal dengan lower urinary tract symptoms (LUTS) yang akan dinilai berdasarkan international prostate symptoms score (IPSS). Metode: Penelitian ini bertujuan untuk mengetahui korelasi volume prostat dengan derajat lower urinary tract symptoms di RSUD dr. Zainoel Abidin Banda Aceh. Pengetahuan tentang volume prostat sangat penting dalam menangani pasien-pasien dengan benign prostatic hyperplasia. Studi ini menggunakan desain Cross Sectional dengan memilih responden secara accidental sampling dengan jumlah sebanyak 31 pasien.Hasil: Data yang didapat kemudian dianalisis dengan uji Spearman. Hasil analisis data univariat menunjukkan bahwa 18 orang pasien (58,1%) memiliki volume prostat Grade III (31-50 cc) dan paling banyak pasien yang memiliki skor derajat LUTS sedang (8-19) yaitu sebanyak 12 pasien (38,7%) dan derajat LUTS berat (20-35) sebanyak 17 pasien (54,8%). Hasil analisis data volume prostat dengan derajat LUTS menunjukkan bahwa nilai p-value = 0,323 (p>0,05).Simpulan: Hasil penelitian ini menunjukkan bahwa volume prostat tidak memiliki korelasi dengan derajat LUTS pasien BPH di RSUD dr. Zainoel Abidin Banda Aceh.

Abstrake-Jurnal Pustaka Kesehatan, vol. 6 (no. 1), Januari 2018 97 Diagnosis of benign prostate hyperplasia (BPH) can be established through transabdominal ultrasonography (TAUS), a noninvasive supportive examination which measure prostate volume (PV), and intravesical prostatic protrusion (IPP Keywords: BPH, international prostate symptom score, transabdominal ultrasonographyDiagnosis dari benign prostate hyperplasia (BPH) dapat ditegakkan melalui pemeriksaan penunjang noninvasif transabdominal ultrasonography (TAUS) yang mengukur prostate volume (PV) dan intravesical prostatic protrusion (IPP). Beratnya gejala obstruksi yang dirasakan oleh pasien dinilai dengan kuesioner international prostate symptom score (IPSS). Penelitian ini bertujuan untuk mengetahui hubungan antara PV dan IPP terhadap IPSS pada pasien BPH. Penelitian analitik observasional dengan desain studi cross sectional yang dilaksanakan di poli bedah urologi RS Bina Sehat Jember, RS Paru Jember dan RS Bhayangkara Bondowoso selama bulan Oktober-November 2016 pada 30 responden. Rata-rata PV 52,01±25,18 mL, rara-rata IPP 12,67±6,69 mm, dan rata-rata skor IPSS 25,60±9,20. Hasil uji korelasi Spearman antara PV terhadap IPSS-Voiding, IPSS-Storage, dan total skor IPSS serta IPP terhadap IPSS-Voiding menunjukkan tidak ada korelasi yang bermakna (p=0,319; p=0,067; p=0,085; p=0,183). Uji korelasi Spearman antara IPP terhadap IPSS-Storage dan total skor IPSS memiliki nilai p=0,012 dan r=0,451 serta p=0,027 dan r=0,404 menunjukkan adanya korelasi yang bermakna dengan kekuatan korelasi sedang. Secara statistik IPP berkorelasi lebih baik terhadap skor IPSS dibandingkan dengan PV.
