Purpose Anterior cervical discectomy with fusion is a common procedure for treating radicular arm pain. Polyetheretherketone (PEEK) plastic is a frequently used material in cages for interbody fusion. Silicon nitride is a new alternative with desirable bone compatibility and imaging characteristics. The aim of the present study is to compare silicon nitride implants with PEEK cages filled with autograft harvested from osteophytes. Methods The study is a prospective, randomized, blinded study of 100 patients with 2 years follow-up. The primary outcome measure was improvement in the Neck Disability Index. Other outcome measures included SF-36, VAS arm pain, VAS neck pain, assessment of recovery, operative characteristics, complications, fusion and subsidence based on dynamic X-ray and CT scan. Results There was no significant difference in NDI scores between the groups at 24 months follow-up. At 3 and 12 months the NDI scores were in favor of PEEK although the differences were not clinically relevant. On most follow-up moments there was no difference in VAS neck and VAS arm between both groups, and there was no statistically significant difference in patients' perceived recovery during follow-up. Fusion rate and subsidence were similar for the two study arms and about 90% of the implants were fused at 24 months. Conclusions Patients treated with silicon nitride and PEEK reported similar recovery rates during follow-up. There was no significant difference in clinical outcome at 24 months. Fusion rates improved over time and are comparable between both groups.

Cervical arthroplasty is an accepted and widely performed surgical intervention with usually favorable outcomes. We report a rare case of a 37-year-old woman who presented with vertebral body osteolysis, 6 years after two-level cervical arthroplasty (C5–C6 and C6–C7). The patient showed no initial complications, but at year 6 she presented with neck and radicular arm pain. Diagnostic imaging revealed osteolysis of the vertebral body C6. The patient subsequently underwent removal of the two prostheses and C6 corpectomy, followed by bridging C5 to C7 and anterior fixation. During surgery, the mantle of the prostheses appeared broken, with the core expulsed anteriorly and embedded in granulomatous scar tissue. Pathologic evaluation of the samples showed partially proliferating fibroblasts, giant cell reaction, and textile particles. Postoperatively, clinical symptoms resolved with no residual deficits. Additional research should be performed to assess the long-term complications of this procedure that should be included in the patient informed consent materials.

Anterior Segmental Decompression and Interbody Fusion for Multi-segmental Cervical Spondylotic Myelopathy in the Elderly

Abstract:To explore the clinical effect of anterior segmental decompression and interbody fusion in treatment of elderly patients with complex cervical spondylotic myelopathy of three segments or more, 50 patients, with titanium plate internal fixation bone fusion underwent anterior cervical decompression and multi segmental cervical spondylosis, were collected. Parameters of operation time, intraoperative blood loss, JOA score 1 week before and after, 12 months after operation, cervical X-ray, CT and MRI before and after operation were measured. Clinical symptoms and the improvement of JOA, internal fixation and bone grafting fusion, cervical standard sagittal Cobb angle, lateral fusion segment of vertebral body height (height of anterior border, HAB) and posterior height (height of posterior border, HPB), were observed. Operation time was 90 ~ 150 minutes, average 120 minutes. The amount of bleeding 150 ~ 450 ml, average 325.5 ml. Mean follow-up time was 15.1 months. 2 weeks after operation, sympathetic symptoms were improved obviously, lower limb muscle strength was increased, physical activity was significantly improved, and anesthesia of double upper limbs disappeared. Except for the 3 cases who had serious cervical spinal cord injury showed no obvious recovery, the other 47 patients showed significant improved neurological recovery. X-ray results of 12 months after operation showed that 50 cases of fused intervertebral, intervertebral height and lordosis maintained good. In general, anterior segmental decompression and bone graft fusion and titanium plate internal fixation operation is a safe and effective operation for treating the three section and above cervical spondylotic myelopathy, worth recommending.
