Since color matching is considered a subjective procedure, accurate shade choice is often the most challenging stage of recreating the natural appearance of teeth. Furthermore, accurate determination of tooth color is imperative for the final outcome of dental restorations. The purpose of this research is to assess the accuracy of color match between diverse shade selection methods throughout a systematic review and meta-analysis. Two independent investigators (L.H. and R.B.) screened the literature in five electronic databases. Randomized controlled trials or in vitro papers studying the effect of using either digital shade selection or visual shade selection on the accuracy of color match were included. A total of 13 manuscripts comprised the meta-analysis. Color difference (ΔE) between restorations where the shade matching was performed by the conventional method was greater than those where the shade matching was performed by computerized methods (p = 0.007). According to the subgroup analysis, only the use of digital photographs for shade matching showed a reduction in the (ΔE) (p < 0.0001), while the use of a spectrophotometer has no advantages over the use of visual shade guide tabs (p = 0.57). On the other hand, global analysis showed that incorrect shade matching was higher when the conventional method using shade guide tabs was used (p < 0.001), irrespective of whether a spectrophotometer or a digital camera was used (p < 0.001). This study concluded that the use of digital photography and spectrophotometric measurements led to fewer color differences and less incorrect shade matching than conventional methods using color shade tabs.

Objectives: While various approaches are available for tooth whitening, the basic concept employs the use of peroxides in the form of gels, which are applied to tooth surfaces. Previous studies have shown that reactive oxygen species acting as potent disinfectants can be produced using boron-doped diamond (BDD) electrodes for the electrolysis of water. With these electrodes being applicable, for example, for endodontic treatment, it was the goal of this pilot study to use such electrodes for tooth whitening.Material and Methods: Two groups (n = 10) of intact clinical crowns were obtained by horizontally cutting off roots of extracted human teeth. The crowns were either bleached by applying a commercially available agent based on 40% hydrogen peroxide or were immersed in saline undergoing electrolysis with BDD electrodes.Whitening of specimens was judged on standardized photographs by examiners with three different levels of experience. Statistical analysis was based on Gwet's AC2 coefficient with quadratic weights, Shapiro-Wilk tests, and two-way analysis of variance of aligned rank transformed data (level of significance set at α = .05).Results: Levels of reliability ranging from fair to substantial were recorded for single persons while the level of reliability ranged between fair and moderate for groups of raters. The level of experience had no significant effect on the ratings (p = .2500).The bleaching method had a significant effect on ratings (p = .0005) with BDD electrodes showing less effect.Conclusions: Bleaching by applying BDD electrodes was possible, but was not as effective as the use of commercially available in-office whitening gel. A potential explanation may be seen in different concentrations of reactive oxygen species.

Objective To evaluate the accuracy and repeatability of intraoral instruments used for shade determination compared to visual shade selection (VSS). Materials and Methods A total of 20 subjects and 10 observers, balanced by gender, participated in the study. Observers performed VSS of the upper right central incisor from each subject. Instrumental shade determination of the same teeth was performed using a spectrophotometer (Easyshade V) and intraoral scanners (CEREC Omnicam, Primescan, Trios 3 and Trios 4). Vita Classical shade nomenclature was used to record the shade designation for all instruments and VSS. The accuracy of the instruments was determined by comparing the instrument readings with the most frequent visual shade selected. The percentage of accuracy was obtained by comparing the number of agreements with the number of comparisons. The percentage of repeatability was obtained by comparing the number of repeated shades with the number of shade measurements in each group. Accuracy was compared using Cochran Q test followed by pairwise comparisons using multiple McNemar's tests with Bonferroni correction. Repeatability of the instruments was evaluated using Cronbach's alpha. Results Omnicam showed a significant lower accuracy than the other instruments (p < 0.05). No statistical difference on repeatability was found among the different instruments (p > 0.05). It was not found any statistical differences for VSS accuracy among the observers (p = 0.437) and between genders (p = 0.867). Conclusion Instrumental repeatability (≥75%) and similar accuracy between the best performed instruments (69%–77.5%) and the observers performance (65%–90%) supports the use of high‐performance instruments for dental shade determination. Clinical Significance Most instruments (Easyshade V, Primescan, Trios 3, and Trios 4) showed similar accuracy performance to observers in the VSS.
