Comparing Child-Pugh, MELD, and FIB-4 to Predict Clinical Outcomes in Hepatitis C Virus-Infected Persons: Results From ERCHIVES

Abstract: FIB-4 score is a better predictor of HD and HCC in HCV-positive persons. A score of <3 is associated with a low risk of HD and HCC 1 and 3 years after HCV diagnosis.

“…Depending on the total points, Child-Pugh score can be divided into grade A (5–6 points), B (7–9 points) and C (10–15 points) [18]. The MELD score was calculated using the formula: 11.2 × ln (international normalized ratio) + 9.57 × ln (creatinine, mg/dL) + 3.78 × ln (bilirubin, mg/dL) + 6.43 [19]. Clinically significant portal pressure (CSPH) was defined as the presence of esophageal varices or in those with a low platelet count (<100 ×10 9 /L) with splenomegaly [20].…”

“…Persons with advanced liver fibrosis at baseline (FIB-4 score >3.25) had a higher incidence, and had a higher risk, of CVD events compared with those with no or minimal fibrosis (FIB-4 score <1. 25). However, at all levels of liver fibrosis, those who received treatment had a consistently lower incidence of CVD events compared with those who did not receive treatment.…”

“…The ERCHIVES is updated regularly to include veterans with a new diagnosis of HCV and has been described in detail in multiple previous publications. [19][20][21][22][23][24][25] For the present study, we identified all persons with confirmed chronic HCV infection (positive for HCV antibody and !1 HCV RNA). We excluded those with human immunodeficiency virus coinfection and hepatitis B virus coinfection.…”

“…Determination of the CP class was performed according to methods published previously [ 25 ]. The model of end-stage liver disease (MELD) score was computed applying the formula: 9.57 × ln(creatinine [mg/dl]) + 11.2 × ln(INR) + 3.78 × ln(bilirubin [mg/dl]) + 6.43 [ 26 ]. The calculation of the ALBI score used this equation: ALBI score = − 0.085 × (albumin [g l − 1 ]) + 0.66 × log 10 (total bilirubin [μmol l − 1 ]), and was further categorized into three different grades: grade 3 (> − 1.39), grade 2 (> − 2.60 to ≤ − 1.39), and grade 1 (≤ − 2.60) [ 10 ].…”
