Comparing anterior gastropexy to no anterior gastropexy for paraesophageal hernia repair: a study protocol for a randomized control trial

Abstract: Background More than half of patients undergoing paraesophageal hernia repair (PEHR) will have radiographic hernia recurrence at 5 years after surgery. Gastropexy is a relatively low-risk intervention that may decrease recurrence rates, but it has not been studied in a prospective manner. Our study aims to evaluate the effect of anterior gastropexy on recurrence rates after PEHR, compared to no anterior gastropexy. Methods This is a two-armed, sing… Show more

“…The trial involves comparing anterior gastropexy to no anterior gastropexy in paraesophageal hernia repair. 42 After the conclusion of the reconstruction, the use of endoscopy is recommended to detect the presence of any abnormalities, such as esophagitis or resistance to endoscope passage, and check for sufficient fundoplication integrity, such as location of the wrap relative to the diaphragmatic hiatus, location of the squamocolumnar junction greater than 1 cm proximal to the wrap zone, and an intact wrap. 43 Jobe et al 44 created 10 criteria that would define a successful fundoplication and should be checked for using endoscopy at the end of the reconstruction.…”
