Aims. To investigate the effect of Vitapex combined with AH-plus paste on inflammation in middle-aged and elderly patients with periodontal-endodontic disease. Materials and Methods. A total of 80 teeth of middle-aged and elderly patients with the periodontal-endodontic disease who were treated in our hospital from June 2020 to June 2021 were selected as the research objects and were divided into comparison group and treatment group with 40 teeth each according to different treatment methods. Among them, the comparison group was filled with zinc oxide clove oil paste gutta-percha, and the treatment group was filled with Vitapex combined with AH-plus paste. The changes in apical cross-section sealing performance, periodontal pocket depth (PD), and gingival index (GI) of the two groups before treatment and after 6 months of follow-up were observed and compared, and X-ray analysis was performed. Results. There was no significant difference in the level of periodontal indexes and serum inflammatory indexes between the two groups before treatment ( P > 0.05 ). After treatment, the levels of PD, BI, PLI, and GI in the two groups were significantly lower than before treatment ( P < 0.05 ). The differences in IL-6, IL-10, TNF-α, and IL-1β after treatment were significant ( P < 0.05 ). There was no significant difference in the root canal area data of each cross-section between the two groups when the root canal was filled with root filling paste at different positions from the apex ( P > 0.05 ). After root canal filling with root canal paste, the cross-sectional area from different parts of the root canal in the treatment group was significantly smaller than comparison group ( P < 0.05 ). After 6 months of treatment, it was found that CEJ-BD and CEJ-AC in the treatment group were significantly lower than those in the comparison group ( P < 0.05 ). Conclusion. Vitapex combined with AH-plus paste filling can effectively treat middle-aged and elderly periodontal-endodontic disease, which provides a certain reference for the clinical treatment of middle-aged and elderly periodontal-endodontic disease.

The push-out bond strength of three root canal materials used in primary teeth: in vitro study

The study aims to compare the bond strength of three primary tooth root canal filling materials to the root canal wall with a push-out test (Calplus, Diapaste, BIOfactor MTA). First, 30 primary central teeth were cut transversely using a water-cooled low-speed diamond saw vertical to the long axis to obtain 2 mm thick discs from the middle third of the roots. Next the materials used were placed on dentin discs and kept in an incubator for 1 week at 37°C and 100% humidity until the hardening mechanism of the root-canal sealer was completed. Finally, a vertical force was placed on each material from apical to coronal with a 0.75 mm diameter stainless steel cylindrical piston without contacting the root canal dentin. The data were analyzed using the SPSS 22.0 program and Mann-Whitney U test was used as a post hoc test. There was a statistically significant difference between the bonding values of different primary tooth root canal sealers to root canal dentin (p < 0.05). Among the maximum binding values, the lowest measurement was in Calplus (0.43 ± 0.28 MPa), and the highest measurement was in BIOfactor MTA (24.24 ± 17.78 MPa) (p < 0.05). BIOfactor MTA has a higher bonding value to root canal dentin than calcium hydroxide-based primary tooth canal sealers.

To compare the efficiency of conventional needle irrigation, passive ultrasonic irrigation, sonic irrigation and XP‐endo Finisher in removing Vitapex paste from root canals. The root canals of human single‐rooted teeth were prepared and obturated with Vitapex paste. After 2 weeks, the 48 teeth were randomly allocated into four groups: conventional needle irrigation (CNI) group, passive ultrasonic irrigation (PUI) group, sonic irrigation (SI) group and XP‐endo Finisher (XP) group. The specimens of four groups were scanned using a micro‐computed tomography after the Vitapex pastes was removed. The results showed that more residual paste was left in the CNI group than in the other three groups (p < 0.05). There was no significant difference among the PUI, SI and XP group (p > 0.05). And, the majority of the residual Vitapex was found in the apical third, with a small amount in the middle third after using any of the three agitated irrigation techniques.
