Background: Traditional septoplasty is being phased out in favor of endoscopic septoplasty. It ensures minimal tissue dissection and fewer surgical problems. Aim of the study: Evaluation the effectiveness of the endoscopic septoplasty for correction of different types of septal deformity (deviation_ spur ) compared to endoscopic assisted conventional septoplasty. Patients and Methods: This prospective study was conducted on 60 patients of both sex, who attached to the out-patient E.N.T clinic at Bab El Sharaya (Sayed Galal) Hospital from April of 2021 to March of 2022 distributed as 2 same groups: Group A included thirty patients submit to totally endoscopic septoplasty and group B involved thirty patients underwent conventional septoplasty assisted by endoscope . All the patients suffering from nasal obstruction due to a significant septal deviation. Results: Both groups had no significant differences in age or gender. While there was a substantial difference in the duration of operation and intraoperative blood loss between the two groups. The length ranged from 20 to 50 minutes in group A and 35 to 65 minutes in group B, while the blood loss ranged from 25 to 65 ml in group A and 45 to 90 ml in group B. In this study, the difference between the endoscopic and conventional groups was statistically insignificant in terms of early postoperative complications like nasal synechiae, septal hematoma, septal perforation, or CSF leakage, as well as the intraoperative flap and late post-operative complications after 3 months in both groups. Conclusion: Both techniques are effective in reducing nasal obstruction and related symptoms with fewer overall complications in the endoscopic approach.

Background: Although conventional septoplasty is widely used to treat nasal septum deviation, it increases morbidity due to poor visualization, poor illumination, the need for nasal packing, and difficulty in evaluating of the exact pathology. These drawbacks are also encountered in endoscopic septoplasty. Our study aimed to compare the treatment and complication outcomes of conventional and endoscopic septoplasty. Methods: The authors searched five electronic databases for relevant clinical trials. The records were screened for eligibility. Data were extracted from the included studies. Outcomes were pooled as risk ratios (RR) or mean differences with 95% CIs using RevMan ver.5.4. Results: Our study included 13 randomized clinical trials with 735 patients. Our analysis revealed that endoscopic septoplasty was significantly (P<0.05) superior to conventional septoplasty for postoperative nasal obstruction relief, intraoperative and postoperative hemorrhage, and mucosal adhesion and synechiae across both long-term and short-term follow-ups. The following pooled RR values were found in short-term follow-up periods: [RR=1.20, 95% CI:=(1.09,1.32)]; [RR=0.27, 95% CI=(0.14,0.54)]; and [RR=0.16, 95% CI=(0.08,0.32)], respectively. Regarding persistent septal deviation and septal tear, endoscopic septoplasty had the upper hand only in short-term follow-up periods [RR=0.30, 95% CI=(0.17,0.53)] and [RR=0.26, 95% CI=(0.15,0.46)], respectively. Conclusion: Our analysis revealed that endoscopic septoplasty was significantly superior to conventional septoplasty in postoperative nasal obstruction relief rate and reducing the risk of intraoperative and postoperative hemorrhage, mucosal adhesion and synechiae, persistent septal deviation, septal tear, and surgery duration.

Background Septoplasty techniques have evolved over the years with endoscopic septoplasty gaining popularity in the recent times. Objective To compare the outcomes of conventional septoplasty with endoscopic septoplasty using Freer’s incision in symptomatic deviated nasal septum. Method It was a prospective, randomized and comparative study done at Department of ENT Head and Neck Surgery, Tribhuvan University Teaching Hospital, Kathmandu, Nepal from July 2018 to August 2019. A total of 70 patients with symptomatic deviated nasal septum were allocated randomly into two groups. Group A underwent conventional septoplasty whilst Group B underwent endoscopic septoplasty using Freer’s incision. Sino-nasal outcome test (SNOT-10) score was recorded pre-operatively and post -operatively between four to six weeks of surgery. Paired and independent ‘t’ test for mean was used as a statistical tool. Result Out of 70 patients, 57(81.43%) were males and 13(18.57%) females. The age group mostly affected was in the third and fourth decades with total 47 patients (67.14%). In the conventional group, the pre-op mean SNOT-10 score was 11.46 (SD±3.6) while post-op mean SNOT-10 score was 2.60 (SD±1.9), the difference being statistically significant (p value 0.00001). Similarly, in the endoscopic group, pre-op mean SNOT-10 score was 12.06 (SD±4.88) and post-op mean SNOT-10 score was 3.37 (SD±2.71) with the difference being statistically significant (p value 0.00001). Comparison of post-operative mean SNOT-10 score between two techniques was not statistically significant (p value 0.17). Conclusion Both conventional and endoscopic septoplasty techniques using Freer’s incision were equally effective in improving symptoms due to deviated nasal septum.
