Background Pathologic scars can lead to itching, erythema, and psychological stress due to cosmetic problems. Bleomycin, one of anticancer agents, has been used for treating keloid or hypertrophic scar. Some studies have shown that bleomycin can induce markedly scar improvement. Aims To evaluate the efficacy of bleomycin compared to corticosteroid as well as other treatments for keloid or hypertrophic scar using meta‐analysis methods. Methods A computerized search was performed in different databases including Cochrane, Embase, and PubMed. Three randomized controlled trials (RCTs) and two controlled clinical trials (CCTs) were included. Then, statistical analyses of extracted outcome data from the studies were calculated using Rex (version 3.0.1; RexSoft Inc). Results Scar improvement was significantly increased in the bleomycin group compared to the triamcinolone acetonide (TAC) group (SMD: 0.59, 95% CI: 0.30‐0.88, P < .0001). In addition, there was also statistically significant difference between the bleomycin group and the 5‐FU group (SMD: 1.37, 95% CI: 0.88‐1.85, P < .0001). Bleomycin increased relatively scar improvement compared to TAC combined with 5‐FU, although there was no statistical difference (SMD: 0.63, 95% CI: −0.59‐1.84, P = .3108). Furthermore, bleomycin demonstrated significantly increased improvement of scar compared to TAC combined with cryotherapy (SMD: 1.11, 95% CI: 0.48‐1.74, P = .0006). Conclusions This meta‐analysis found that bleomycin was more effective for treating keloid or hypertrophic scar than other treatments including TAC, 5‐FU, TAC combined with 5‐FU, and TAC combined with cryotherapy. However, further comprehensive studies, including randomized controlled trials, are needed to perform objective analysis.

Background: Skin hyperpigmentation usually results from an increased number, or activity, of melanocytes. The degree of pigmentation of skin depends on the amount and type of melanin, degree of skin vascularity, presence of carotene, and thickness of the stratum corneum. Common causes of hyperpigmentation include post-inflammatory hyperpigmentation, melasma, solar lentigines, ephelides (freckles), and café-au-lait macules. Some skin tumors can be hyperpigmented as basal cell carcinoma (BCC), squamous cell carcinoma (SCC) and malignant melanoma (MM). Stem cell factor (SCF) is a growth factor and its interaction with its receptor, c-kit, is well known to be critical to the survival of melanocytes. Methods: This study was carried out on 60 patients complaining of hyperpigmented skin lesions (20 melasma, 20 solar lentigines, and 20 freckles) and 36 patients with skin tumors (14 BCC, 12 SCC, and 10 MM). Punch skin biopsies were taken from the previous lesions. Immunohistochemical staining of these samples was done using the stem cell factor (SCF). Results: There was positive expression of SCF in all cases of melasma, solar lentigines and freckles with significant increase in the intensity of expression in the lesional areas than the non-lesional ones (P=0.004). There was also a statistically significant increase in the expression of SCF in BCC and melanoma tumor cells. Conclusion: SCF has a great role in skin hyperpigmented disorders and this can be used as a target for the developing of new antipigmentary lines of treatment by inhibiting SCF. SCF can also be involved in the emergence of some skin tumors.

Background Keloids are reported to be amongst the common conditions seen in the pigmented skin of African origin with an estimate of about 15 times more compared with other races. They seldom resolve spontaneously, have a poor response to treatment and pose enormous impact on quality of life (QoL) in affected individuals. Aesthetic discomfort, depression, low self‐esteem and some degree of isolation increase the burden of the disease. Methods This hospital‐based cross‐sectional study was conducted at the Regional Dermatology Training Centre in Northern Tanzania. A total of 117 patients with keloid older than 18 years attended the skin clinic from October 2017 to April 2018. Data were collected using disease‐specific structured questionnaire and all were assessed using the Dermatology Life Quality Index (DLQI) Questionnaire. Results The mean DLQI for this population was 8.3 ± 7.4 reflecting a moderate impairment. The QoL seemed to be influenced by the number of keloids, duration of disease and the presence of symptoms. It was mainly seen in young adults with a median age of 29 years (range: 18–88 years). There was no significant sex predilection. Trauma as an inciting event was reported in 42%. Conclusion Keloid patients with pigmented skin of African descent in Northern Tanzania have a moderate impairment in the QoL.
