Comparative Population Analysis of Klebsiella pneumoniae Strains with Extended-Spectrum β-Lactamases Colonizing Patients in Rehabilitation Centers in Four Countries

Abstract: The international project MOSAR was conducted in five rehabilitation centers; patients were screened for rectal carriage of extended-spectrum ␤-lactamase (ESBL)-producing members of the Enterobacteriaceae. Among 229 Klebsiella pneumoniae isolates, four clonal groups (CG) or complexes (CC) prevailed: CG17 in France, CG101 in Italy, CG15 in Spain, and CC147 in Israel. ESBLs, mainly CTX-Ms, were produced by 226 isolates; three isolates expressed AmpC-like cephalosporinases. High genetic diversity of K. pneumoniae… Show more

“…Novel alleles and STs identified in this study were submitted to the database for coding. CGs and CCs were determined by using the eBURST program (http://eburst.mlst.net) against the MLST database with 1,948 STs (as of 3 July 2015) as described previously (9,13,14). Pulsed-field gel electrophoresis (PFGE) of XbaI-digested genomic DNA was performed on a CHEF-DR III apparatus (Bio-Rad Laboratories, Hercules, CA) according to the instruction manual.…”

“…The epidemic drug-resistant clones, called "high-risk" clones (11), may play an important role in the dissemination of antimicrobial resistance. The major international high-risk clones identified by MLST belong to sequence type 11 (ST11), ST15, ST147, and ST258 (11)(12)(13)(14)(15)(16). ST11 and ST258 were included in clonal group (CG) 258 (11,14), while ST15 and ST147 were grouped in CG15 and CG147 (11,13,14), respectively.…”

“…K. pneumoniae ST336 is a single locus variant of ST17 in the clonal complex 17, whose major clones are ST16, ST17 and ST20 [27][28][29][30]. K. pneumoniae isolates of clonal complex 17 are well recognized contributors to the spread of CTX-M-15 worldwide [31] and ST336 is among the less prevalent yet international clones that have been associated with the spread of various ESBL enzymes, including CTX-M-15, and even carbapenemase resistance [23,32,33].…”

“…The group included 29 CMY-2-positive clinical isolates, two E. coli transconjugants/transformants with CMY-2-like enzymes (E. coli A15 or DH5␣), and seven non-CMY-producing isolates (13)(14)(15)(16)(17)(18)(19)(20)(21). E. coli ATCC 25922 and Klebsiella pneumoniae ATCC 13883 were used as negative controls.…”
