Antimicrobial resistance patterns of the most frequently isolated Gram-positive bacteria in selected Latin American hospitals were evaluated under the auspices of the SENTRY Antimicrobial Surveillance Program. The strains were consecutively collected (one per patient) between January 2003 and December 2008 and tested by reference broth microdilution methods at the monitoring central laboratory (JMI Laboratories, North Liberty, Iowa, USA). A total of 12,324 Gram-positive cocci were analyzed. The organisms were isolated from bloodstream (53.2%) and skin and skin structure infections (16.4%). Resistance to oxacillin (MRSA) was observed in 40.0% of Staphylococcus aureus, varying from 32.7% in Brazil to 49.7% in Chile. Resistance to erythromycin (90.1%), clindamycin (84.4%), and levofloxacin (86.8%) was very high in MRSA. Vancomycin, linezolid and daptomycin were all very active against S. aureus strains tested (>99.9-100.0% susceptible), but daptomycin (MIC(50/90), 0.25/0.5 microg/ml) was four- to eight-fold more potent than three comparators. Vancomycin resistance increased from 5.0% in 2003 to 15.5% in 2008 among enterococci (VRE); the most significant increase occurred among isolates from Brazilian medical centers (from 6.9 to 31.1%). Daptomycin was the most active antimicrobial tested against enterococci in general (MIC(50/90) microg/ml; 100.0% susceptible), followed by linezolid (MIC(50/90), 1/2 microg/ml; 99.9% susceptible), teicoplanin (MIC(50 )and MIC(90 )of <2 microg/ml; 91.3% susceptible), vancomycin (MIC(50/90), 1/8 microg/ml; 89.6% susceptible). In summary, daptomycin and linezolid showed excellent in vitro activity against Gram-positive organisms (12,324) collected in Latin American hospitals, including MRSA, VRE and other multidrug-resistant organisms.

BackgroundThe Tigecycline Evaluation and Surveillance Trial (T.E.S.T.) is a global surveillance study of antimicrobial susceptibility. This study reports data from Gram-negative isolates collected from centers in Latin America between 2004 and 2010.MethodsConsecutive bacterial isolates were tested at each center using broth microdilution methodology as described by the Clinical Laboratory Standards Institute (CLSI). Susceptibility was determined using the CLSI interpretive criteria. For tigecycline the US Federal Drug Administration (FDA) criteria were used.ResultsA total of 16 232 isolates were analyzed. Susceptibility to imipenem, meropenem, and tigecycline was >95% against both non-extended-spectrum β-lactamase (ESBL) and ESBL producing Escherichia coli. Susceptibility to amikacin was also >95% for non-ESBL E. coli. 24.3% of E. coli were ESBL producers, ranging from 11.2% (58/519) in Colombia to 40.3% (31/77) in Honduras. Greater than 90% of non-ESBL Klebsiella pneumoniae were susceptible to tigecycline, carbapenems and amikacin. 35.3% of K. pneumoniae were ESBL producers, ranging from 17.2% (36/209) in Venezuela to 73.3% (55/75) in Honduras, with only imipenem and tigecycline maintaining >90% susceptibility. Greater than 90% of Klebsiella oxytoca, Enterobacter spp., and Serratia marcescens were susceptible to amikacin, carbapenems and tigecycline. The highest rates of susceptibility against Acinetobacter baumannii were seen for minocycline (89.4%) and imipenem (62.5%), while 95.8% of the A. baumannii isolates displayed an MIC ≤2 μg/mL for tigecycline.ConclusionsIn this study carbapenems and tigecycline remain active against Enterobacteriaceae and A. baumannii; however, there is cause for concern with carbapenem non-susceptible isolates reported in all countries included in this study.

Community acquired pneumonia (CAP) is an important cause of morbidity and mortality around the world, with high treatment costs due to hospitalization and complications (adverse events due to medications, antibiotic resistance, healthcare associated infections, etc.). It has been proposed administration of short courses and early switch of intravenous administration to oral therapy to avoid costs and complications. There are recommendations about these topics in national and intemational guidelines, based on clinical trials which do not demónstrate diffe-rences in mortality and complications when there is an early change from intravenous administration to the oral route. There are no statistically significant differences in safety and resolution of the disease when short and long treatment schemes were compared. In this review we present the most important guidelines and clinical studies, taking into account the pharmacological differences between different medications. It is considered that early switch from intravenous to oral administration route and use of short cycles in CAP is safe and brings benefits to patients and institutions.
