To study the effect of bioactive glass bone substitute granules (S53P4) on bacterial adhesion and biofilm formation on other simultaneously used implant materials and the role of the hypoxic conditions to the adhesion. Bacterial and biofilm formation were studied on materials used both in middle ear prostheses and in fracture fixtures (titanium, polytetrafluoroethylene, polydimethylsiloxane and bioactive glass plates) in the presence or absence of S53P4 granules. The experiments were done either in normal atmosphere or in hypoxia simulating atmospheric conditions of middle ear, mastoid cavity and sinuses. We used two collection strains of Staphylococcus aureus and Staphylococcus epidermidis. In the presence of bioglass and hypoxic conditions the adhesion of the planktonic bacterial cells was decreased for most of the materials. The biofilm formation was decreased for S. epidermidis on titanium and polydimethylsiloxane in both atmospheric conditions and on bioglass plates in normoxia. For S. aureus the biofilm formation was decreased on bioglass plates and polytetrafluoroethylene in normoxia. Hypoxia produces a decrease in the biofilm formation only for S. aureus on polytetrafluoroethylene and for S. epidermidis on bioglass plates. However, in none of the cases bioactive glass increased the bacterial or biofilm adhesion. The presence of bioglass in normoxic and hypoxic conditions prevents the bacterial and biofilm adhesion on surfaces of several typical prosthesis materials in vitro. This may lead to diminishing postoperative infections, however, further in vivo studies are needed.

Es sollen bei möglichst geringem Gewicht, hoher mechanischer Festigkeit und Biokompatibilität gute Schallübertragungsqualitäten gegeben sein. Eine hohe Oberflächenenergie, die zur gewünschten Epithelialisierung der Gehörknöchelchenkette beiträgt, wird ebenso gefordert, wie Stabilität gegenüber Infektionen und körpereigene Abbauvorgänge. Das Material muss ein funktionsangepasstes Design ermöglichen und gleichzeitig intraoperativ bearbeitbar sowie problemlos lagerfähig sein. Eine Induktion von Knochenneubildung ist im selben Maße unerwünscht, wie die Verursachung von Artefakten bei radiologischen Untersuchungen. Für Kernspintomographien spielt zudem eine geringe Magnetisierbarkeit eine wichtige Rolle, da sonst Innenohrschädigungen drohen [8, 15, 16]. Im Jahre 1963 wurde von Schubert erst-malig eine Stapesprothese aus Gold eingeführt [23]. Seitdem haben sich in den letzten 20 Jahren Metalle und hierbei vor allem Gold, Titan und Stahl als Implantatmaterial in der Mittelohrchirurgie im klinischen Einsatz bewährt [8]. Um die Integration des Materials bereits im Vorfeld günstig beeinflussen zu können, ist zunächst die genaue Kenntnis seiner physikochemischen Eigenschaften Voraussetzung, da sie bei in Lebewesen implantierten Materialien verantwortlich für die biologischen Effekte auf Gewebe, Zellen und Proteine sind [30]. Es kann davon ausgegangen werden, daß es bereits unmittelbar nach Implantation zur Adsorption körpereigener Proteine an der Materialoberfläche kommt [31]. Durch diesen Vorgang wird eine gewebsspezifische zelluläre Reaktion in Gang gesetzt, [14, 30], deren komplizierter Verlauf durch die ständigen Veränderungen an der Materialoberfläche, z.B. durch Korrosions-, Degradations-, und Auslaugprozesse, bestimmt wird [4, 21, 32]. So ist die Antigenbildung durch Bindung von aus o. g. Prozessen frei gewordenen Elementen an Proteine ebenso wie die Veränderung makromolekularer Bestandteile der interstitiellen Flüssigkeit gut untersucht [2]. Ein im Gegensatz dazu noch weitgehend hypothetischer Ansatz zur Erklärung der Biokompatibilität stellt die Wirkung von Proteinen dar, die aus der interstitiellen Flüssigkeit an der Oberfläche Gold hat sich als Biomaterial zur Rekonstruktion der Gehörknöchelchenkette etabliert. Dennoch finden sich bei der Anwendung und in der Literatur Hinweise auf Unverträglichkeitsreaktionen mancher Patienten. Abstract: Because a certain roughness of the piston surface is necessary for the development of a stable membrane between the piston and the edge of the stapedotomy opening, the titanium prosthesis is considered to be the most suitable for stapes surgery of the three pistons examined.
