Comparative efficacy of salbutamol by pressurized aerosol and wet nebulizer in acute asthma.

Abstract: 1 Cumulative dose-response curves (FEVy) 3 Detailed analysis of the data according to a theoretical dose-response relationship showed that the nebulizer-aerosol increment was unrelated to asthma severity.

“…The following factors influenced our choice of MDIS: (1) We examined the literature regard¬ ing metered-dose inhalers and freon toxicity9,12"17 and concluded that the risk to the patient would be minimal if the following guidelines were used: (1) no more than two puffs at a time before patient réévaluation; (2) puff-to-puff waiting time of at least 1 minute; (3) an absolute ceiling of 20 puffs in any treat¬ ment period (Twenty puffs would repre¬ sent a maximum dose of 350 mL of freon over 20 minutes. For a 5-year-old child this should represent less than 3% ofthe minute volume.…”

“…The bronchodilating effect of inhaled Pj-^'irnulants has previously been shown in acute cumulative dose-response studies to level out after five inhalations (20,37,39,40). 7 (xg/ml, and 2) to evaluate these acute effects by superimposing five terbutaline inhalations (1.25 mg terbutaline sulphate).…”

“…In a large series of FEV, readings from 792 men with bronchitic symptoms, Fletcher, Peto, Tinker & Speizer (1976) also recommended use of the maximum because in their study it was more reliable. However, the standard deviation In the course of a recent study of bronchodilator therapy in acute asthma (Tarala, Madsen & Paterson, 1980) we addressed this question of mean or maximum FEVy readings and concluded our data did not provide any evidence for use of other than the mean as a location representative. FEV, readings were taken in sets of three from 17 severe acute asthmatics admitted to hospital and given salbutamol by pressurised aerosol (Ventolin, 2 puffs = 0.2 mg, 259 measurements) and by wet nebuliser (Mini-neb, Inspiron, 5 mg, 240 measurements).…”
